Indications
Amdopril is prescribed as a substitution therapy for patients already stable on separate doses of perindopril and amlodipine. It is used to treat hypertension and/or stable coronary heart disease. Amdopril should not be initiated as a first-line treatment.
Pharmacology
Perindopril, a pro-drug, converts to perindoprilat, which inhibits ACE, leading to reduced angiotensin II levels. This results in vasodilation, reduced aldosterone secretion, and increased diuresis. Amlodipine, a dihydropyridine calcium channel blocker, prevents calcium ion influx into vascular smooth and cardiac muscle, primarily affecting vascular smooth muscle and thereby reducing blood pressure.
Dosage & Administration
Amdopril is used for patients previously managed with separate doses of perindopril (5 or 10 mg) and amlodipine (5 or 10 mg). The recommended dosage is one tablet daily, ideally in the morning and before meals. Dose adjustments should be made based on clinical judgment and individual patient needs, using separate perindopril and/or amlodipine products if necessary.
Interactions
Combining Amdopril with baclofen may enhance its antihypertensive effect, requiring careful monitoring of blood pressure and renal function. Concurrent use with nitrates or other vasodilators may exacerbate hypotension.
Contraindications
Amdopril is contraindicated in patients with hypersensitivity to perindopril, amlodipine, ACE inhibitors, or dihydropyridines. It should not be used during pregnancy or lactation.
Side Effects
Common side effects include edema, dizziness, headache, dysgeusia, fatigue, and gastrointestinal disturbances. Allergic reactions, hypotension, and muscle spasms may also occur.
Pregnancy & Lactation
Amdopril should not be started during pregnancy or lactation. Alternative antihypertensive treatments with established safety profiles should be considered.
Precautions & Warnings
Patients with lactose intolerance should avoid Amdopril due to its lactose content. Regular monitoring of serum electrolytes is essential, especially when used with diuretics.
Use in Special Populations
Amdopril is not recommended for children due to a lack of safety and efficacy data.
