Reset

Indications

Reset is effective for treating various conditions such as fever, the common cold, influenza, headaches, toothaches, earaches, body aches, myalgia, neuralgia, dysmenorrhea, sprains, colic pain, back pain, postoperative pain, postpartum pain, inflammatory pain, and post-vaccination discomfort in children. It is also recommended for alleviating rheumatic and osteoarthritic pain and joint stiffness.

Pharmacology

Reset contains paracetamol, known for its analgesic and antipyretic properties, with minimal anti-inflammatory effects. It is believed to work primarily in the central nervous system (CNS) by increasing the pain threshold through the inhibition of cyclooxygenase enzymes (COX-1, COX-2, and COX-3), which are integral to prostaglandin synthesis. Paracetamol is a para-aminophenol derivative that is widely recognized as a safe and fast-acting analgesic, well-tolerated and free from many side effects associated with aspirin.

Dosage & Administration

Tablet:

• Adults: 1-2 tablets every 4 to 6 hours, not exceeding 4 grams (8 tablets) daily.
• Children (6-12 years): ½ to 1 tablet 3 to 4 times daily. For extended treatment, do not exceed 2.6 grams per day.

Extended Release Tablet:

• Adults & Children over 12 years: 2 tablets every 6 to 8 hours, maximum of 6 tablets in 24 hours. Tablets should be swallowed whole, not crushed.

Syrup/Suspension:

• Children under 3 months: 10 mg/kg body weight (reduce to 5 mg/kg if jaundiced), 3 to 4 times daily.
• 3 months to under 1 year: ½ to 1 teaspoonful 3 to 4 times daily.
• 1-5 years: 1-2 teaspoonfuls 3 to 4 times daily.
• 6-12 years: 2-4 teaspoonfuls 3 to 4 times daily.
• Adults: 4-8 teaspoonfuls 3 to 4 times daily.

Suppository:

• Children 3-12 months: 60-120 mg, 4 times daily.
• Children 1-5 years: 125-250 mg 4 times daily.
• Children 6-12 years: 250-500 mg 4 times daily.
• Adults & Children over 12 years: 0.5-1 gram 4 times daily.

Paediatric Drop:

• Children Up to 3 months: 0.5 ml (40 mg).
• 4 to 11 months: 1.0 ml (80 mg).
• 1 to 2 years: 1.5 ml (120 mg). Do not exceed more than 5 doses daily for a maximum of 5 days.

Paracetamol tablets utilizing Actizorb technology dissolve up to five times faster than standard tablets, providing rapid relief and safety for those unable to tolerate aspirin or other analgesics.

• Adults and children (aged 12 years and over): Take 1 to 2 tablets every 4 to 6 hours as needed. Do not exceed 8 caplets in 24 hours.
• Children (7 to 11 years): Take ½-1 tablet every 4 to 6 hours as needed. Do not exceed 4 caplets in 24 hours. Not recommended for children under 7 years.

Interaction

Patients taking barbiturates, tricyclic antidepressants, or alcohol may experience a reduced ability to metabolize large doses of Reset. Alcohol consumption can increase the hepatotoxicity associated with an overdose of Reset. Long-term use of anticonvulsants or oral steroid contraceptives can induce liver enzymes, potentially preventing effective therapeutic levels of Reset due to increased first-pass metabolism or clearance.

Contraindications

Reset should not be used by individuals with known hypersensitivity to paracetamol.

Side Effects

While side effects are generally mild, some hematological reactions, including thrombocytopenia, leucopenia, pancytopenia, neutropenia, and agranulocytosis, have been reported. Occasional cases of pancreatitis, skin rashes, and other allergic reactions may also occur.

Pregnancy & Lactation

Epidemiological studies have shown that paracetamol, when used at recommended doses, does not produce adverse effects during pregnancy. However, patients should follow their healthcare provider’s guidance regarding its use. Although paracetamol is excreted in breast milk, it is not in clinically significant amounts, and available data do not contraindicate breastfeeding.

Precautions & Warnings

Caution is advised when administering Reset to patients with severe renal or hepatic impairment. The risk of overdose is heightened in individuals with non-cirrhotic alcoholic liver disease. Do not exceed the recommended dosage, and patients should be instructed to avoid concurrent use of other Reset-containing products. Reset should be prescribed only when necessary.

Overdose Effects

In adults, liver damage may occur after taking 10 grams or more of Reset. Ingestion of 5 grams or more can lead to liver damage in patients with certain risk factors, including long-term treatment with enzyme-inducing medications, excessive alcohol consumption, or conditions that deplete glutathione.

Symptoms: Early signs of overdose may include pallor, nausea, vomiting, loss of appetite, and abdominal pain within the first 24 hours. Liver damage may become evident 12 to 48 hours post-ingestion, with potential abnormalities in glucose metabolism and metabolic acidosis. Severe cases may lead to hepatic failure, encephalopathy, hemorrhage, hypoglycemia, cerebral edema, and potentially death. Acute renal failure with acute tubular necrosis may occur even without severe liver damage. Immediate treatment is critical for managing an overdose. Activated charcoal should be considered within one hour of ingestion. Plasma paracetamol concentration should be measured at 4 hours or later post-ingestion (earlier concentrations may not be reliable). N-acetylcysteine may be administered up to 24 hours after ingestion, with optimal protective effects occurring within 8 hours. If necessary, intravenous N-acetylcysteine should be given according to the established dosage schedule, while oral methionine may be an alternative in remote areas. Management of severe hepatic dysfunction beyond 24 hours after ingestion should involve consultation with a poison control center or liver unit.
