Indications
Flupentixol and Melitracen tablets are prescribed for various conditions including anxiety, depression, apathy, psychogenic depression, depressive neuroses, masked depression, psychosomatic affections accompanied by anxiety and apathy, menopausal depressions, dysphoria, and depression in alcoholics and drug addicts.
Pharmacology
This medication comprises two well-known compounds: flupentixol, a neuroleptic with anxiolytic and antidepressant properties in small doses, and melitracen, a bipolar thymoleptic with activating properties in low doses. Together, they provide an antidepressant, anxiolytic, and activating effect. Flupentixol reaches maximal serum concentration about 4 hours after oral administration, while melitracen reaches it in approximately 4 hours as well. Their biological half-lives are approximately 35 and 19 hours, respectively.
Dosage & Administration
For adults, the usual dosage is 2 tablets orally daily in the morning and noon, which can be increased to 2 tablets in the morning for severe cases. Elderly patients are typically prescribed 1 tablet in the morning. A maintenance dose of 1 tablet orally in the morning is usually recommended, with additional sedative treatment advised for insomnia or severe restlessness during the acute phase, as per the physician’s guidance.
Interactions
This medication may enhance the response to alcohol, barbiturates, and other CNS depressants. Concurrent use with MAO inhibitors may lead to hypertensive crises. Neuroleptics and thymoleptics may diminish the antihypertensive effect of certain drugs and enhance the effects of adrenaline and noradrenaline.
Contraindications
This medication is contraindicated during the immediate recovery phase after myocardial infarction, in cases of defects in bundle-branch conduction, untreated narrow-angle glaucoma, acute alcohol, barbiturate, and opiate intoxications, and in patients who have received MAO inhibitors within two weeks. It is also not recommended for excitable or overactive patients due to its activating effects.
Side Effects
Side effects are rare at recommended doses but may include transient restlessness and insomnia.
Pregnancy & Lactation
Use of this medication is preferably avoided during pregnancy and lactation.
Precautions & Warnings
Patients previously treated with tranquillizers with sedative effects should have them withdrawn gradually.
Overdose Effects
Overdose symptoms may include intoxication, especially of anticholinergic nature, and, rarely, extrapyramidal symptoms. Symptomatic and supportive measures should be administered promptly, including gastric lavage and activated charcoal. Respiratory and cardiovascular systems should be supported, and caution should be exercised regarding the use of epinephrine.
