Indications
Lipired is indicated for the treatment of hyperlipidemias of types IIa, IIb, III, IV, and V in patients who have not responded adequately to diet and other appropriate measures.
Pharmacology
Fenofibrate, a fibric acid derivative, rapidly converts to its active form, fenofibric acid, after oral ingestion. Fenofibric acid reduces total cholesterol, LDL cholesterol, Apo-lipoprotein B, total triglycerides, and VLDL, while increasing HDL and apo-proteins apoAI and apoAII. Additionally, fenofibrate lowers serum uric acid levels by increasing urinary excretion, with enhanced absorption in its micronized form.
Dosage & Administration
The initial dose of Fenofibrate for adults with hypercholesterolemia or mixed hyperlipidemia is 200 mg per day. For hypertriglyceridemia, the initial dose ranges from 67 to 200 mg per day, with dosage adjustments based on patient response. Fenofibrate should be taken with meals to optimize bioavailability, or as directed by a registered physician.
Interaction
Lipired may potentiate the anticoagulant effects of warfarin and improve glucose tolerance when administered with antidiabetic drugs. It may also increase the nephrotoxicity of cyclosporine. Caution is advised when co-administering with HMG-CoA reductase inhibitors to avoid the risk of rhabdomyolysis, although low-dose statins with Lipired are generally well tolerated.
Contraindications
Fenofibrate is contraindicated in patients with hypersensitivity to fenofibrate, severe renal or hepatic impairment, existing gall bladder disease, breastfeeding mothers, and those with photosensitivity to ketoprofen.
Side Effects
Common side effects may include gastrointestinal symptoms, pruritus, urticaria, impotence, headache, dizziness, vertigo, fatigue, and hair loss, with potential for myotoxicity.
Pregnancy & Lactation
Fenofibrate is not recommended for use in pregnant women.
Precautions & Warnings
Special care is required in patients with renal disease, as progressive increases in serum creatinine concentration or failure to follow dosage guidelines may lead to myotoxicity. Liver function tests are recommended every three months during the first year of treatment.
Overdose Effects
There is no specific treatment for overdose with Lipired. Supportive care, including monitoring of vital signs and clinical status, is recommended. Hemodialysis is not considered effective due to Lipired’s high plasma protein binding.
