Indications
I-Guard is prescribed for the management of ocular infections affecting the conjunctiva and/or cornea caused by organisms susceptible to I-Guard. These include Staphylococcus aureus, Streptococcus pneumoniae, E. coli, H. influenzae, Klebsiella/Enterobacter species, Moraxella lacunata, and Neisseria species.
Pharmacology
Chloramphenicol, the active ingredient in I-Guard, is a broad-spectrum bacteriostatic antibiotic that inhibits bacterial protein synthesis by interfering with the transfer of activated amino acids from soluble RNA to ribosomes.
Dosage & Administration
For both adults and children, instill 1 or 2 drops into the conjunctival sac 4 to 6 times daily during the first 72 hours, followed by every 4 hours thereafter. Treatment should typically continue for about 7 days but should not exceed 3 weeks without reassessment by a healthcare provider.
Always follow your physician’s advice regarding medication use.
Interactions
The concurrent use of chymotrypsin may be inhibited when administered alongside I-Guard.
Contraindications
I-Guard should not be used in patients with a known hypersensitivity to chloramphenicol or any of its components.
Side Effects
Short-term topical use of I-Guard has not been associated with systemic adverse reactions. However, common side effects may include burning, stinging, conjunctival hyperemia, blood dyscrasia, allergic or inflammatory responses, as well as vesicular and maculopapular dermatitis.
Pregnancy & Lactation
The safety of I-Guard during pregnancy and lactation has not been established. It should only be used if deemed essential by a healthcare professional.
Precautions & Warnings
I-Guard should not be used for minor infections or as a prophylactic measure. Avoid repeated courses and prolonged use, as prolonged ophthalmic application may lead to blood dyscrasias, including granulocytopenia, thrombocytopenia, and moderate anemia.
Overdose Effects
Accidental ingestion of I-Guard is unlikely to result in toxicity due to its low antibiotic content.
