Description

Indications

Citofer is used for the following medical conditions:

  • Control of Serum Phosphorus: In adult patients with chronic kidney disease on dialysis.
  • Iron Deficiency Anemia: For adult patients with chronic kidney disease who are not on dialysis.

Use Citofer as prescribed by a registered healthcare professional.


Pharmacology

Citofer contains ferric citrate, which binds dietary phosphate in the gastrointestinal tract. This forms an insoluble ferric phosphate compound that is excreted through the stool, reducing phosphate absorption and lowering serum phosphate levels. Additionally, ferric iron is converted to ferrous iron, absorbed, and incorporated into hemoglobin, supporting iron levels in the body.


Dosage & Administration

For Hyperphosphatemia in Chronic Kidney Disease on Dialysis:

  • Starting dose: 2 tablets, three times daily with meals.
  • Dose can be adjusted by 1-2 tablets as required, with a maximum of 12 tablets per day.

For Iron Deficiency Anemia in Chronic Kidney Disease (Not on Dialysis):

  • Starting dose: 1 tablet, three times daily with meals.
  • Dose may be adjusted as needed to achieve the hemoglobin target, with a maximum of 12 tablets per day.

Use in Children & Adolescents: The safety and effectiveness of Citofer have not been established for children or adolescents.


Interactions

Citofer may interact with oral antibiotics like Doxycycline and Ciprofloxacin. These should be taken separately from Citofer.


Contraindications

  • Iron Overload: Not suitable for patients with hemochromatosis or other iron overload syndromes.

Side Effects

Common side effects include:

  • Discolored feces
  • Diarrhea, constipation, nausea, and vomiting
  • Abdominal pain and cough
  • Hyperkalemia (high potassium levels)

Pregnancy & Lactation

There is insufficient data on the use of Citofer during pregnancy and lactation. Consult a healthcare professional before use.


Precautions & Warnings

  • Iron Overload: Iron absorption may lead to elevated iron stores. Patients might need a dose reduction or cessation of intravenous iron therapy.
  • Accidental Overdose: Keep out of reach of children. Accidental overdose can be fatal. In case of overdose, immediately contact a healthcare provider or poison control center.

Overdose Effects

There is no specific data on Citofer overdose. However, in patients with chronic kidney disease, the maximum studied dose was 2,520 mg of ferric iron per day.
