Indications
Torsid serves as a remedy for addressing edema linked with congestive heart failure, renal, or hepatic ailments. Additionally, Torsid is prescribed for managing hypertension, either individually or alongside other antihypertensive medications.
Pharmacology
Torsemide operates within the thick ascending portion of the loop of Hen
le, inhibiting the Na+/K+/2Cl^- carrier system. This action boosts the urinary excretion of sodium, chloride, and water without significantly affecting glomerular filtration rate, renal plasma flow, or acid-base equilibrium.
Dosage & Administration
- For heart failure-related edema: Initiate with Torsemide 10 mg or 20 mg once daily, adjusting upwards if necessary.
- For edema due to chronic renal failure: Commence with Torsemide 20 mg once daily, adjusting as needed.
- For hepatic cirrhosis-associated edema: Begin with Torsemide 5 mg or 10 mg once daily, in combination with an aldosterone antagonist or potassium-sparing diuretic, adjusting the dosage accordingly.
- For hypertension: Start with 5 mg once daily, increasing to 10 mg if required. Additional antihypertensive agents may be added if needed.
Interaction
Care should be taken when administering Torsid concurrently with high doses of salicylates, cholestyramine, ACE inhibitors, or Angiotensin receptor blockers due to potential interactions.
Contraindications
Torsemide is not suitable for patients with known hypersensitivity to the drug or those who are anuric or experiencing hepatic coma.
Pregnancy & Lactation
Torsemide falls under Pregnancy Category B. Its excretion in human milk is uncertain, thus caution is advised during lactation.
Precautions & Warnings
- Excessive diuresis may lead to symptomatic dehydration, reduced blood volume, and hypotension.
- Torsid can cause electrolyte and metabolic abnormalities, including hypokalemia, hyponatremia, and hypochloremic alkalosis.
Therapeutic Class
Torsid belongs to the category of loop diuretics.
