Description

Indications

  • Xynovir is indicated in combination with other antiretroviral agents for the treatment of HIV-1 infection in adults.
  • It is also used for managing chronic hepatitis B in adult patients.

Pharmacology

  • Mechanism of Action: Xynovir contains Tenofovir, which inhibits the activity of HIV reverse transcriptase by competing with deoxyadenosine 5-triphosphate. As an analog of naturally occurring deoxynucleotides, Tenofovir integrates into the viral DNA chain, preventing its synthesis.
  • Pharmacokinetics: Tenofovir is a water-soluble prodrug with approximately 25% oral bioavailability. After administration, peak serum concentrations are typically reached in 1-2 hours. Tenofovir binds minimally to plasma proteins, and its half-life is about 17 hours. The drug is eliminated through a combination of glomerular filtration and active tubular secretion.

Dosage & Administration

  • For HIV-1 or Chronic Hepatitis B Treatment:
    • Adults: Take 1 tablet of 245 mg Tenofovir once daily, orally, without regard to food.
    • Pediatric Patients (12 years or older, ≥35 kg): 1 tablet of 245 mg once daily, orally.
  • Adjustments may be required for patients with renal impairments.

Interactions

  • Didanosine: Caution is required when using Xynovir alongside Didanosine. Close monitoring for adverse reactions is recommended.
  • Atazanavir: Patients taking Atazanavir and Xynovir together should be closely monitored for potential adverse effects.
  • Lopinavir / Ritonavir: Co-administration with these drugs requires monitoring for any side effects related to Xynovir.

Contraindications

  • Xynovir should not be used in patients with:
    • Severe kidney disease, liver disease, or bone density issues.
    • Pregnant women (use only if absolutely necessary) or breastfeeding mothers.

Side Effects

  • Common side effects of Xynovir include:
    • Headache, diarrhea, nausea, and weakness.
    • Less common side effects: indigestion, chest pain, drowsiness, high cholesterol, kidney issues, or mental health concerns (depression, anxiety).

Pregnancy & Lactation

Precautions & Warnings

  • Lactic acidosis and severe hepatomegaly with steatosis have been reported in combination therapy with Xynovir.

  • Fat redistribution and immune reconstitution syndrome may also occur in patients undergoing HIV treatment.

Use in Special Populations

  • Renal Impairment: Adjust Xynovir dosage for patients with renal impairment.
  • Pediatric: Not recommended for children under 12 years of age.
  • Geriatric: No significant differences noted, but caution is advised due to age-related organ function decline.
