Indications
Xpos is prescribed for patients aged 13 years and older to treat various fungal infections, including:
- Invasive aspergillosis in cases resistant to or intolerant of Amphotericin B or Itraconazole
- Fusariosis in cases resistant to or intolerant of Amphotericin B
- Chromoblastomycosis and mycetoma in cases resistant to or intolerant of Itraconazole
- Coccidioidomycosis in cases resistant to or intolerant of Amphotericin B, Itraconazole, or Fluconazole, or in patients intolerant of these medications.
Additionally, Xpos is indicated for prophylaxis against invasive fungal infections in:
- Patients undergoing remission-induction chemotherapy for Acute Myelogenous Leukemia (AML) or Myelodysplastic Syndromes (MDS) with prolonged neutropenia and high risk of fungal infections
- Hematopoietic Stem Cell Transplant (HSCT) recipients undergoing high-dose immunosuppressive therapy for graft versus host disease and at high risk of fungal infections.
Pharmacology
Xpos (Posaconazole) works by inhibiting the synthesis of ergosterol, a crucial component of fungal cell membranes. This action disrupts membrane integrity, leading to fungal cell death.
Dosage & Administration
Xpos Tablets can be taken with or without food. Swallow tablets whole with water; do not crush, chew, or break them.
- Loading Dose: 300 mg (3 Tablets) twice daily on the first day.
- Maintenance Dose: 300 mg (3 Tablets) once daily from the second day onward.
Duration of therapy depends on disease severity, recovery from immunosuppression, and clinical response.
Interactions
Xpos inhibits CYP3A4 and should be used cautiously with medications metabolized by this enzyme, such as Midazolam, Triazolam, Alprazolam. Avoid combining Xpos with Rifamycin antibacterials (Rifampicin, Rifabutin) and certain anticonvulsants (Phenytoin, Carbamazepine, Phenobarbital, Primidone, Efavirenz) due to potential interactions.
Contraindications
Xpos is contraindicated in individuals with hypersensitivity to Posaconazole or other azole antifungals. Avoid co-administration with ergot alkaloids, certain CYP3A4 substrates, and medications that may prolong QTc interval.
Side Effects
Common side effects observed with Xpos include diarrhea, nausea, fever, vomiting, headache, coughing, and hypokalemia.
Pregnancy & Lactation
Avoid using Xpos during pregnancy unless benefits outweigh risks. Discontinue breastfeeding when starting treatment with Xpos.
Use in Special Populations
- Pediatric Use: Not recommended for patients under 13 years.
- Geriatric Use: No dose adjustment necessary.
- Hepatic Impairment: Use with caution; monitor liver function tests.
- Renal Impairment: Dose adjustment not needed in mild to moderate impairment; monitor closely in severe impairment.
