Indications
This triple therapy regimen is prescribed for the eradication of Helicobacter pylori (H. pylori), a common cause of chronic gastritis, and for the treatment of duodenal and gastric ulcers.
Composition
Each Vonokit blister pack contains:
- 1 tablet of Vonoprazan 20 mg (as Vonoprazan Fumarate INN)
- 2 capsules of Amoxicillin 500 mg (as Amoxicillin Trihydrate BP)
- 1 tablet of Clarithromycin 500 mg (as Clarithromycin USP)
Each Vonocab Trio blister pack contains:
- 2 tablets of Vonoprazan 20 mg (as Vonoprazan Fumarate INN)
- 4 capsules of Amoxicillin 500 mg (as Amoxicillin Trihydrate BP)
- 2 tablets of Clarithromycin 500 mg (as Clarithromycin USP)
Each Vonolend kit includes:
- 2 tablets of Vonoprazan 20 mg
- 4 capsules of Amoxicillin 500 mg
- 2 tablets of Clarithromycin 500 mg
Each Helicon V kit consists of:
- 1 tablet of Vonoprazan INN 20 mg
- 2 capsules of Amoxicillin BP 500 mg
- 1 tablet of Clarithromycin USP 500 mg
Pharmacology
H. pylori is a primary factor in gastritis and peptic ulcers. The combination of Vonoprazan (20 mg), Amoxicillin (500 mg), and Clarithromycin (500 mg) is a powerful regimen for eliminating this bacterium.
Vonoprazan works by inhibiting gastric acid secretion, blocking the H+/K+ ATPase enzyme, commonly known as the acid pump. Unlike other proton-pump inhibitors, Vonoprazan doesn’t need acidic activation, making it effective in both active and resting states. Amoxicillin is a well-known antibacterial agent, while Clarithromycin is a macrolide antibiotic that works synergistically with the other components. Suppressing stomach acid increases the efficacy of these antibiotics against H. pylori.
Dosage & Administration
The recommended dosage for this regimen is:
- 20 mg Vonoprazan
- 1000 mg Amoxicillin
- 500 mg Clarithromycin
Take twice daily (12 hours apart) for 14 days, with or without food.
Special Considerations:
- Geriatric patients may have an increased risk of torsades de pointes due to Clarithromycin.
- Avoid in patients with severe renal or moderate to severe hepatic impairment.
Drug Interactions
Exercise caution when co-administering this therapy with medications like Tacrolimus, Cyclosporine, Clopidogrel, and certain antivirals or antifungals.
Contraindications
This regimen is contraindicated in individuals with known hypersensitivity to Vonoprazan, Amoxicillin, Clarithromycin, or other related antibiotics.
Side Effects
Common side effects (≥2%) include taste disturbances, diarrhea, headaches, abdominal pain, and hypertension.
Pregnancy & Lactation
Due to potential risks identified in animal and human studies, this therapy is not recommended for pregnant or breastfeeding women unless no alternative exists.
Warnings & Precautions
- Hypersensitivity: Discontinue use immediately if any serious allergic reaction occurs.
- QT Prolongation: Avoid in patients with QT interval prolongation or those taking medications that extend the QT interval.
- Hepatotoxicity: Discontinue use if liver issues arise.
Overdose
In case of overdose, consult a healthcare provider immediately.
Storage
Store below 30°C in a dry place, away from light and out of children’s reach.
