Indications
Tylace serves as an adjunctive therapy for specific clinical conditions characterized by the presence of thick and viscous mucoid or mucopurulent secretions. These conditions include chronic bronchopulmonary diseases such as chronic obstructive pulmonary disease (COPD), emphysema with bronchitis, chronic asthmatic bronchitis, and bronchiectasis, as well as acute bronchopulmonary diseases like asthma with bronchial mucus plugging, bronchitis, bronchopneumonia, tracheobronchitis, bronchiolitis, and pulmonary complications associated with cystic fibrosis or surgery.
Pharmacology
Acetylcysteine acts as a mucolytic agent by reducing the viscosity of secretions, likely through the cleavage of disulfide bonds in mucoproteins. Additionally, it exhibits antisecretory effects, aiding in the clearance of respiratory ducts and facilitating breathing. Acetylcysteine also possesses antioxidant properties, neutralizing free radicals and serving as a precursor to glutathione, an essential antioxidant in both intra and extracellular environments. By providing antioxidant action, it counteracts exogenous and endogenous oxidants, which can exacerbate respiratory inflammations.
Dosage & Administration
- Effervescent Tablet or Dispersible Tablet: For adults and children above 6 years old, one effervescent tablet of Acetylcysteine 600 mg daily is recommended, preferably in the evening. Treatment duration ranges from 5 to 10 days in acute cases and may extend to several months in chronic conditions, following physician advice.
- Effervescent Granules: Adults and children above 6 years old may take 1 sachet of Acetylcysteine 200 mg or 2 sachets of Acetylcysteine 100 mg, 2-3 times daily. For children aged 2-6 years, the dosage is 1 sachet of Acetylcysteine 100 mg, administered 2-4 times daily, depending on age.
- Nebuliser Solution: Dilute the 20% solution to a lower concentration with appropriate solutions for inhalation. Dosage varies for adults and children based on age and condition severity, administered every 6-8 hours as needed.
Interaction
The thiol group of Tylace may affect the efficacy of certain antibiotics and increase the inhibitory effects of nitroglycerine on thrombocyte aggregation and vasodilation. It’s advisable to administer Tylace and amoxicillin at least 2 hours apart to avoid potential interactions.
Contraindications
Tylace is contraindicated in individuals with known hypersensitivity to Acetylcysteine and in patients with phenylketonuria due to the presence of aspartame.
Side Effects
Possible side effects of Tylace include immune reactions such as anaphylactic shock and severe skin reactions, albeit very rare. Other less severe side effects may include headache, increased heart rate, gastrointestinal discomfort, pruritus, urticaria, nausea, vomiting, abdominal pain, fever, hypotension, diarrhea, and tinnitus.

Precautions & Warnings
Strict monitoring is necessary for patients with bronchial asthma during therapy, with immediate suspension of treatment if bronchospasm occurs. Caution is advised in patients with or a history of peptic ulcers. Patients on a controlled sodium diet should consider the sodium content in Viscotin (156.9mg per dose). Mixing Tylace solution with other drugs is not recommended.
Overdose Effects
Overdose of Tylace may lead to symptoms such as nausea, vomiting, or diarrhea.
