Description

Indications

Tycil is prescribed for the treatment of infections caused by susceptible ß-lactamase negative bacteria, including:

  • Ear, Nose, and Throat Infections: Otitis media, sinusitis, tonsillitis, pharyngitis, laryngitis
  • Lower Respiratory Tract Infections: Pneumonia, acute and chronic bronchitis, lung abscess, empyema, bronchiectasis
  • Skin and Soft Tissue Infections: Cellulitis, carbuncles, furunculosis, infected wounds, abscesses
  • Genito-Urinary Tract Infections: Pyelonephritis, cystitis, urethritis
  • Venereal Disease: Acute uncomplicated gonorrhoea
  • Dental Abscesses: Short-term therapy
  • Triple Therapy: In combination with Clarithromycin and Lansoprazole, for H. pylori infection and duodenal ulcer disease, and to lower duodenal ulcer recurrence risk.

Pharmacology

Amoxicillin is a broad-spectrum penicillin effective against many Gram-positive and Gram-negative bacteria. It works by inhibiting cell wall biosynthesis, is stable in gastric acid, and is rapidly absorbed. Peak plasma concentration is achieved within 1-2 hours post-administration, with widespread distribution throughout body tissues and fluids.

Dosage

  • Adults: Start with 250 mg three times daily, increasing to 500 mg three times daily for severe infections.
  • Children (up to 10 years): Begin with 125 mg three times daily, increasing to 250 mg three times daily for severe infections.
    • Severe or Recurrent Purulent Respiratory Infections: 3 gm every 12 hours.
    • Otitis Media: 1 g three times daily for adults; 40 mg/kg body weight daily in three divided doses for children (maximum 3 g daily).
    • Pneumonia: 500-1000 mg three times daily.
    • Dental Abscess: 3 gm, repeated after 10-12 hours.
    • Urinary Tract Infections: 3 gm, repeated after 10-12 hours.
    • Gonorrhoea: Single dose of 2-3 gm with Probenecid 1 gm (Probenecid is contraindicated in children under 2 years).
    • Renal Impairment: Dosage adjustment may be necessary.

Administration

Reconstituted suspension can be mixed with milk, fruit juice, or water and should be consumed immediately.

Interactions

  • Probenecid: May increase and prolong Tycil blood levels.
  • Oral Contraceptives: Tycil may affect gut flora, reducing the effectiveness of estrogen/progesterone contraceptives.

Contraindications

Tycil is contraindicated in patients with penicillin hypersensitivity.

Side Effects

Typically mild and transient, including diarrhea, indigestion, and occasional rash. Rarely, pseudo-membranous colitis may occur.

Pregnancy & Lactation

Amoxicillin is FDA category B. Use during pregnancy only if necessary. It is excreted in breast milk; caution is advised for lactating mothers.

Precautions & Warnings

Monitor for superinfections. Discontinue Tycil and initiate appropriate therapy if superinfections occur.

Reconstitution

  • Intramuscular: Add 2.5 ml water for injection to the vial.
  • Intravenous: Dissolve in 10 ml water for injection.
