Indications

Telmilok serves as a therapeutic solution for various conditions, including:

• Hypertension: Effective treatment for essential hypertension in adult patients.
• Cardiovascular Prevention: Reduces cardiovascular morbidity in adults with:
  • Atherothrombotic cardiovascular disease (prior history of coronary heart disease, stroke, or peripheral arterial disease).
  • Type 2 diabetes mellitus accompanied by documented target organ damage.

Pharmacology

Telmisartan, a non-peptide angiotensin-II receptor antagonist, plays a crucial role in regulating the renin-angiotensin system. By selectively blocking the AT1 receptor, Telmisartan impedes the vasoconstrictor and aldosterone-secreting effects of angiotensin-II, thus contributing to blood pressure regulation and cardiovascular health.

Dosage & Administration

Dosage should be personalized for each patient. Typically, Telmisartan tablets are initiated at a dose of 40 mg once daily, with blood pressure response correlating with dosage (ranging from 20 to 80 mg). Optimal effects are usually observed within 4 weeks. Adjustment of dosage may be necessary for specific patient groups, such as the elderly or those with renal impairment.

Interaction

Care should be taken when co-administering Telmilok with NSAIDs, as it may increase the risk of renal impairment and diminish the antihypertensive effect. Additionally, combining Telmilok with aliskiren in diabetic patients should be avoided due to potential adverse effects.

Contraindications

Patients with known hypersensitivity to Telmilok or its components should avoid its use.

Side Effects

Common side effects of Telmilok tablets in hypertensive patients include sinusitis, back pain, and diarrhea. For patients at cardiovascular risk, intermittent claudication and skin ulcers may occur.

Pregnancy & Lactation

Telmilok is classified as Pregnancy Category C (first trimester) and D (second and third trimesters). Nursing mothers should carefully weigh the risks and benefits before using Telmilok, considering its potential effects on the nursing infant.

Precautions & Warnings

Special caution should be exercised to prevent fetal or neonatal exposure. Patients with impaired hepatic or renal function require close monitoring. Concomitant use of an ACE inhibitor and angiotensin receptor blocker should be avoided.

Overdose Effects

Overdose symptoms may include hypotension, dizziness, and tachycardia. Prompt medical attention is necessary in cases of overdose to prevent complications such as bradycardia or acute renal failure.
