Indications
Siesta is indicated for:
- Emotional Disturbances: Management of acute anxiety, tension, interpersonal difficulties, agitation, insomnia, and anxiety-related depressive reactions.
- Cardiovascular and Respiratory Disturbances: Treatment of pseudoangina pectoris, pericardial anxiety, tachycardia, emotional hypertension, dyspnea, and hyperventilation.
- Gastrointestinal Issues: Relief for irritable bowel syndrome, epigastric pain, spasms, bloating, and diarrhea.
- Urinary Tract Disorders: Addresses frequency, irritable bladder, and dysmenorrhea.
- Psychosomatic Conditions: Management of psychogenic headaches, asthma, and gastric/duodenal ulcers.
- Chronic Organic Diseases: Provides support for emotional responses related to long-term illnesses.
Pharmacology
Bromazepam, a potent psychotropic medication, effectively reduces anxiety and tension at lower doses, while exhibiting sedative and muscle-relaxant properties at higher doses. It binds to the GABA-A receptor, enhancing its inhibitory effects without affecting other neurotransmitters.
Dosage
- Standard Dosage: For outpatient therapy, the typical dose ranges from 1.5 to 3 mg, taken up to three times daily. Start with a low dose and gradually increase to the optimal level.
- Severe Cases: In hospital settings, doses may range from 6 to 12 mg, taken two or three times daily, not exceeding 8 to 12 weeks. Longer treatment may be necessary with careful patient re-evaluation.
- Elderly and Debilitated Patients: Require lower doses due to increased sensitivity.
- Children: Generally not recommended, but if necessary, doses should be adjusted based on body weight (approximately 0.1-0.3 mg/kg).

Interaction
Combining Siesta with other centrally acting medications (e.g., antidepressants, hypnotics, narcotics) may intensify its sedative effects. Avoid alcohol, as it can amplify the sedative properties of Siesta.
Contraindications
Bromazepam is contraindicated in patients with known hypersensitivity to the drug, severe respiratory or hepatic insufficiency, and sleep apnea syndrome.
Side Effects
Common side effects may include fatigue, drowsiness, muscle weakness, reduced alertness, confusion, headache, and ataxia. These effects are often transient, particularly at the beginning of therapy. Anterograde amnesia may occur at therapeutic doses.
Pregnancy & Lactation
The safety of bromazepam during pregnancy has not been established. Due to excretion in breast milk, its use is discouraged during lactation.
Precautions & Warnings
Benzodiazepines, including Siesta, may lead to physical and psychological dependence, especially in individuals with a history of alcohol use. Gradual dosage reduction is recommended to minimize withdrawal symptoms. Caution is advised while driving or operating machinery.
