Description
Racetril functions as a targeted intestinal antisecretory agent. It effectively reduces excessive water and electrolyte secretion in the intestines caused by cholera toxins or inflammation, without impacting baseline secretory activities.
After oral administration, Racetril is quickly absorbed and metabolized into thiorphan, an enkephalinase inhibitor found in cell membranes, particularly in the small intestine lining. Enkephalinase is responsible for breaking down both external peptides and naturally occurring peptides like enkephalins. Racetril prevents the breakdown of enkephalins, extending their action in the intestine, which helps to reduce fluid loss and hypersecretion. Its anti-diarrheal effect takes effect swiftly without altering the duration of bowel transit.
Pharmacology
Racecadotril enhances the action of natural opioids (enkephalins) by inhibiting enkephalinase on cell membranes. Enkephalins work through δ receptors (delta opioid receptors) to increase chloride absorption by blocking adenylate cyclase.
- Onset: 30 minutes post-administration, marked by plasma enkephalinase inhibition.
Dosage & Administration
Adults: Begin with one capsule, then continue with one capsule three times daily, ideally before meals.
Children (from 3 months of age): Use Racetril granules with oral rehydration. Dosage is based on weight:
- Infants <9 kg: 10 mg sachet three times daily.
- Infants 9–13 kg: Two 10 mg sachets three times daily.
- Children 13–27 kg: One 30 mg sachet three times daily.
- Children >27 kg: Two 30 mg sachets three times daily.
Always take medication as advised by a licensed physician.
Interaction
No known drug interactions. Concurrent use with Loperamide or Nifuroxazide does not alter Racetril’s pharmacokinetics.
Contraindications
Do not use in cases of hypersensitivity to Racecadotril or any formulation ingredients.
Side Effects
Potential side effects include headaches, erythema multiforme, urticaria, and angioedema.
Pregnancy & Lactation
Due to insufficient clinical data, Racecadotril is not recommended for use during pregnancy or breastfeeding.
Precautions & Warnings
Racetril does not replace standard rehydration therapies. The presence of blood in stools or fever may indicate an invasive bacterial infection or another serious condition. Racetril is not suitable for antibiotic-associated diarrhea. Use with caution in patients with liver or kidney impairment.
Overdose Effects
No overdose cases have been reported. In studies, single doses up to 20 times the therapeutic dose (2 g) were well tolerated in adults.
