Indications
The combination of Amoxicillin, Clarithromycin, and Rabeprazole is prescribed for eradicating H. pylori in active chronic gastric, duodenal, and gastric ulcers.
Pharmacology
Amoxicillin exhibits bactericidal action by inhibiting cell wall biosynthesis, leading to bacterial death. Clarithromycin inhibits protein synthesis in susceptible organisms by binding to the donor site on the 50S subunit of the bacterial ribosome. Rabeprazole, a proton-pump inhibitor, suppresses gastric acid secretion by blocking the gastric H+,K+ ATPase.
Dosage & Administration
Take each strip twice daily for 7 days. In severe cases, continue twice daily for 14 days.
Interaction
Rabeprazole has minimal interactions with drugs metabolized by the cytochrome P450 system. Clarithromycin may increase serum theophylline concentrations when used concomitantly with theophylline.
Contraindications
Avoid if there’s a known hypersensitivity to any component of the drug.
Side Effects
Possible adverse reactions include nausea, vomiting, diarrhea, myalgia, confusion, headache, and skin reactions.
Pregnancy & Lactation
Use during pregnancy should be based on the potential benefit justifying the risk. Caution is advised during lactation. Clarithromycin should be avoided in pregnancy except in exceptional circumstances.
Precautions & Warnings
Serious hypersensitivity reactions may occur with Amoxicillin. Use cautiously in elderly patients and those with severe hepatic impairment.
Overdose Effects
Overdose experiences with Rabeprazole are limited. Symptomatic and supportive measures are recommended in cases of overdose with Amoxicillin. Clarithromycin overdose may lead to gastrointestinal symptoms.
