Indications
Pregaben CR film-coated tablets are primarily indicated for the management of various conditions, including:
- Neuropathic Pain: Pain associated with diabetic peripheral neuropathy and spinal cord injury.
- Postherpetic Neuralgia (PHN): To reduce pain following shingles.
- Seizures: Adjunctive therapy for partial-onset seizures in patients aged 1 month and older.
- Fibromyalgia: For the management of widespread pain.
Use only under the guidance of a registered medical professional.
Pharmacology
Pregabalin, a structural analogue of GABA (gamma-aminobutyric acid), works by binding to the alpha2-delta subunit of voltage-gated calcium channels in the central nervous system. While the precise mechanism isn’t fully understood, studies suggest its binding helps alleviate pain and seizures.
Dosage
Pregaben CR dosage should be tailored to the individual patient’s needs. The general recommendations include:
- Neuropathic Pain (Diabetic Peripheral Neuropathy): Start at 50 mg three times daily, with an increase to 300 mg/day as needed.
- Postherpetic Neuralgia: Begin with 75 mg twice daily, adjustable up to 600 mg/day for those needing higher doses.
- Fibromyalgia: Start with 150 mg/day, potentially increasing to 450 mg/day for optimal effect.
- Spinal Cord Injury Pain: Recommended starting dose is 150 mg/day, with increases to 600 mg/day as required.
Note: Always follow your healthcare provider’s instructions for precise dosage adjustments.
Administration
Pregaben CR should be taken orally, with or without food. The controlled-release tablets should be taken after an evening meal and swallowed whole—do not split, crush, or chew. If a dose is missed, take it as soon as remembered, unless it’s almost time for the next dose. If discontinuing, taper the medication gradually over a minimum of 1 week to avoid withdrawal symptoms.
Drug Interactions
Pregaben CR is not expected to cause significant pharmacokinetic drug interactions, making it a relatively safe choice when taken with other medications. However, always inform your healthcare provider about any current medications you are using to avoid potential risks.
Contraindications
Pregaben CR is contraindicated for patients who have known hypersensitivity to pregabalin or any of its components.
Side Effects
Common side effects may include:
- Adults: Dizziness, somnolence, dry mouth, weight gain, and difficulty concentrating.
- Pediatric Patients (for seizure management): Increased appetite and weight gain.
Consult your healthcare provider if you experience any side effects.
Precautions & Warnings
- Angioedema: Seek emergency treatment if swelling of the throat or neck occurs.
- Suicidal Thoughts: Pregabalin, like other antiepileptics, may increase the risk of suicidal thoughts or behavior.
- CNS Depression: Can cause dizziness and somnolence; use caution when driving or operating machinery.
- Withdrawal: Gradually taper Pregaben CR over at least 1 week to prevent withdrawal symptoms.
- Edema: Pregaben CR may cause peripheral edema, requiring monitoring during treatment.
Pregnancy & Lactation
Pregabalin should only be used during pregnancy if absolutely necessary, as it can pose risks to the fetus. Additionally, small amounts of pregabalin may be present in breast milk, so breastfeeding is not recommended during treatment.
Storage Conditions
Store Pregaben CR in a cool, dry place (below 30°C), away from direct light and moisture. Keep out of reach of children to prevent accidental ingestion.
Therapeutic Class
Pregaben CR belongs to the class of anti-epileptic drugs, used primarily as adjunct therapy for seizures and pain management.
