Pharmacology
Antacid Action: Bismuth Subsalicylate creates a protective coating over the ulcer surface, shielding it from gastric acid and pepsin. It enhances the secretion of mucus and bicarbonate while neutralizing hydrochloric acid (HCl), resulting in the formation of bismuth oxychloride and salicylic acid, which further reduces HCl levels.
Anti-Diarrheal Action: This medication promotes the absorption of water and electrolytes through the intestinal wall. In cases of infectious diarrhea, it binds to toxins produced by E. coli, disrupts cellular integrity, and lyses H. pylori, preventing its adherence to the intestinal wall.
Anti-Inflammatory Action: Upon hydrolysis to salicylic acid, Bismuth Subsalicylate inhibits prostaglandin G/H synthase ½, effectively reducing inflammation and forming a protective layer over irritated tissues to slow fluid expulsion.
Dosage & Administration
- Adults (16 years and older): 30 mL using the dosing cup provided or 6 x 5 mL spoonfuls
- Children (10-14 years): 15 mL (3 teaspoons)
- Children (5-9 years): 7.5 mL (1.5 teaspoons)
- Children (3-4 years): 5 mL (1 teaspoon)
- Children under 3 years: Consult a healthcare professional.
Elderly Patients: No special precautions are necessary.
Patients with Renal Insufficiency: Use with caution.
Repeat dosing may be done every ½ to 1 hour as needed, not exceeding 8 doses in a 24-hour period.

Drug Interactions
Peptocid contains salicylates; therefore, caution is advised when used with blood-thinning medications (anticoagulants) or oral medications for diabetes, gout, or arthritis.
Contraindications
Bismuth Subsalicylate is not recommended for patients allergic to aspirin, salicylates, or any components of this formulation. Keep all medications out of reach and sight of children.
Side Effects
Common side effects include black stools and a black tongue. Peptocid should not be used if symptoms are severe or persist for more than two days. Avoid exceeding the recommended dosage, and remember to shake the bottle before use. For oral use only.
Pregnancy & Lactation
There is insufficient data on the safety of Bismuth Subsalicylate for pregnant women. Animal studies do not adequately assess its effects on pregnancy, fetal development, and postnatal outcomes. The potential risks for human use are unknown. Bismuth Subsalicylate should only be used during lactation if absolutely necessary.
Precautions & Warnings
Avoid concurrent use with aspirin or other salicylates. Patients taking anticoagulants, diabetes medications, or gout treatments should exercise caution.
Overdose Effects
Overdose of Peptocid may lead to acute encephalopathy characterized by confusion, myoclonic movements, tremors, dysarthria, and balance issues. Other symptoms may include gastrointestinal disturbances, skin reactions, discoloration of mucous membranes, and renal dysfunction due to acute tubular necrosis. Treatment may involve gastric lavage, purgation, and hydration. Chelating agents may be effective shortly after ingestion, and hemodialysis may be required. Symptoms may mimic salicylate overdose, including dizziness, tinnitus, sweating, nausea, and headaches. Discontinue use of Peptocid if these symptoms occur. Overdose management follows the same protocol as salicylate overdose.
