Indications
Pantonix is indicated for conditions where the suppression of gastric acid secretion is beneficial, including:
- Peptic Ulcer Diseases: Effective for the treatment of various peptic ulcers.
- Gastroesophageal Reflux Disease (GERD): Aids in managing GERD symptoms.
- NSAID-Induced Ulcers: Provides relief from ulcers caused by non-steroidal anti-inflammatory drugs (NSAIDs).
- Helicobacter pylori Eradication: Used in combination with antibiotics for the effective eradication of H. pylori.
- Zollinger-Ellison Syndrome: Helps manage this rare condition characterized by excessive stomach acid production.

Pharmacology
Pantoprazole functions as a proton pump inhibitor (PPI), targeting the final step in gastric acid production. It works by covalently binding to the H+/K+ ATPase enzyme system located on the surface of gastric parietal cells. This mechanism results in the inhibition of both basal and stimulated gastric acid secretion for over 24 hours, regardless of the stimulating factors.
Dosage & Administration
Oral Administration:
- Benign Gastric Ulcer: 40 mg once daily in the morning for 4 weeks; may continue for another 4 weeks if not fully healed.
- Gastroesophageal Reflux Disease: 20-40 mg daily in the morning for 4 weeks; if symptoms persist, may continue for an additional 4 weeks. Maintenance dose: 20 mg daily, adjustable to 40 mg daily if necessary.
- Duodenal Ulcer: 40 mg once daily in the morning for 2 weeks; extend for another 2 weeks if required.
- H. pylori-Associated Duodenal Ulcer: Recommended dosage is 40 mg twice daily alongside antimicrobial therapy: Amoxicillin 1 g and Clarithromycin 500 mg, or Clarithromycin 250 mg and Metronidazole 400 mg, both administered twice daily for one week.
- NSAID-Associated Ulcer Prophylaxis: 20 mg daily for patients requiring long-term NSAID treatment.
- Zollinger-Ellison Syndrome: Start with 80 mg once daily, adjusting based on response (maximum 40 mg daily for elderly patients); doses above 80 mg should be divided into two doses.
IV Injection:
- Duodenal and Gastric Ulcer: 40 mg once daily for 7-10 days.
- GERD with Erosive Esophagitis History: 40 mg once daily for 7-10 days.
- Peptic Ulcer Rebleeding Prevention: Administer 80 mg IV, followed by an infusion of 8 mg/hour for 72 hours.
- Acid Aspiration Prophylaxis: 80 mg IV every 12 hours for 24 hours, followed by 40 mg every 12 hours.
- Long-term Management of Zollinger-Ellison Syndrome: 80 mg IV every 12 hours; may increase to every 8 hours if necessary, with dosage adjusted according to acid output.

Interaction
Clinical studies have not indicated significant drug interactions with Pantonix.
Contraindications
Pantonix should not be administered to individuals with known hypersensitivity to any of its ingredients.
Side Effects
Pantonix is generally well-tolerated for both short-term and long-term use. Common side effects may include headache and diarrhea, while less frequent side effects can include abdominal pain, flatulence, rash, insomnia, and hyperglycemia.
Pregnancy & Lactation
Pantoprazole is classified as a US FDA Pregnancy Category B medication. While no adequate studies have been conducted in pregnant women, it should only be used if necessary. Pantoprazole is excreted in human milk, so healthcare providers should weigh the benefits of the medication against the risks of breastfeeding.
Precautions & Warnings
Patients are advised not to split, chew, or crush Pantonix tablets. Prolonged use may lead to malabsorption of cyanocobalamin (Vitamin B12) or an increased risk of osteoporosis-related conditions.
Overdose Effects
There are no known symptoms of overdose in humans. Due to its high protein binding, Pantoprazole is not easily dialyzable. In case of overdose, symptomatic and supportive management is recommended, as no specific antidote is available.

Reconstitution
IV Injection Directions: Pantonix lyophilized powder should only be administered intravenously. To prepare, add 10 ml of 0.9% Sodium Chloride Injection to the vial containing the powder. Administer the solution slowly over 2 to 5 minutes. The reconstituted solution can be stored at room temperature (up to 30°C) for a maximum of 4 hours.
IV Infusion Directions: Reconstitute Pantonix with 10 ml of 0.9% Sodium Chloride Injection, then dilute with 0.9% Sodium Chloride or 5% Dextrose to achieve a final volume of 100 ml. Store the reconstituted solution at room temperature (up to 30°C) for a maximum of 4 hours before dilution. The mixed solution should be used within 24 hours of initial reconstitution.
