Indications
Mucomist DT is recommended as an adjunct therapy for managing thick and viscous mucus or mucopurulent secretions. It is beneficial in chronic bronchopulmonary conditions such as chronic obstructive pulmonary disease (COPD), emphysema with bronchitis, chronic asthmatic bronchitis, and bronchiectasis. It is also used for acute bronchopulmonary conditions including asthma with bronchial mucus plugging, bronchitis, bronchopneumonia, tracheobronchitis, bronchiolitis, and pulmonary complications related to cystic fibrosis or surgery.
Pharmacology
Mucomist DT contains Acetylcysteine, a potent mucolytic agent that decreases mucus viscosity by breaking down disulfide bonds in mucoproteins. It also possesses antisecretory effects, aiding in the clearance of respiratory passages and improving breathing. Additionally, Acetylcysteine acts as an antioxidant, neutralizing harmful free radicals and boosting glutathione levels, which are crucial for protecting against oxidative stress in respiratory inflammation.
Dosage & Administration
- Effervescent or Dispersible Tablets: For adults and children over 6 years, dissolve one 600 mg tablet daily (preferably in the evening) in a small amount of water. For acute conditions, treatment lasts 5 to 10 days, while chronic conditions may require longer use as advised by a physician.
- Effervescent Granules:
- Adults and children over 6 years: 1 sachet (200 mg) or 2 sachets (100 mg) 2-3 times daily.
- Children (2-6 years): 1 sachet (100 mg) 2-4 times daily, depending on age.
- Nebulizer Solution: Dilute the 20% solution with Sodium Chloride or sterile water as needed:
- Adults: 5-10 ml of 10% or 20% solution every 6-8 hours as needed.
- Children:
- 1-11 months: 1-2 ml of 20% or 2-4 ml of 10% solution every 6-8 hours as needed.
- 12 months-11 years: 3-5 ml of 20% or 6-10 ml of 10% solution every 6-8 hours as needed.
- Under 12 years: 5-10 ml of 10% or 20% solution every 6-8 hours as needed.
Diagnostic Bronchograms: Use 1-2 ml of 20% or 2-4 ml of 10% solution via nebulization or intratracheal instillation before the procedure.
Nebulization Tent or Croupette: Requires large volumes, up to 300 ml, to maintain a heavy mist for the treatment period. Administration can be intermittent or continuous.
Direct Instillation: 1-2 ml of a 20% solution may be used hourly, especially for tracheostomy care.

Interaction
Acetylcysteine in Mucomist DT can affect the efficacy of certain antibiotics, including ampicillin, tetracycline, and macrolides. It is recommended to space the use of Mucomist DT and these antibiotics by 2 hours. Concurrent use with nitroglycerine may enhance its effects on platelet aggregation and vasodilation.
Contraindications
Mucomist DT is contraindicated in individuals with known hypersensitivity to Acetylcysteine or those with phenylketonuria, due to its aspartame content.
Side Effects
Possible side effects include headache, increased heart rate, stomatitis, pruritus, urticaria, nausea, vomiting, abdominal pain, fever, dizziness, and ear noises. Severe reactions like anaphylaxis and bronchospasm may occur in rare cases. Consult your healthcare provider for a comprehensive list of side effects.
Pregnancy & Lactation
Use Mucomist DT during pregnancy or lactation only after consulting a physician or pharmacist.
Precautions & Warnings
Monitor patients with bronchial asthma closely during treatment; discontinue if bronchospasm occurs. Use with caution in individuals with a history of peptic ulcer. The product contains sodium (156.9 mg per dose), which should be considered for those on a sodium-restricted diet. A sulfurous odor is normal and does not indicate product spoilage. Avoid mixing with other medications.
Overdose Effects
In case of overdose, symptoms may include nausea, vomiting, or diarrhea.
