Indications
Metalone Plus eye drops are recommended for the following conditions:
- Acute and chronic allergic non-infectious conjunctivitis and keratitis, particularly with significant swelling and intravascular injection.
- Non-infectious inflammation of the anterior segment of the eye, including anterior uveitis, episcleritis, and scleritis.
- Post-operative irritation following strabismus, cataract, and glaucoma surgeries, often in conjunction with antimicrobial therapy.
Pharmacology
Fluorometholone operates by promoting the production of phospholipase A2 inhibitory proteins, which regulate the synthesis of key inflammatory mediators such as prostaglandins and leukotrienes by inhibiting the release of their common precursor, arachidonic acid. Tetrahydrozoline, an alpha agonist, works by constricting conjunctival blood vessels, effectively reducing eye redness.
Dosage & Administration
For Adults: Administer 1 drop into the conjunctival sac 2 to 3 times daily. For severe cases, the frequency may be increased to 1 drop per hour for the first 1 to 2 days.
For Children: Safety and efficacy have not been established for children under 2 years of age.

Interactions
Tetrahydrozoline may interact with antidepressants that inhibit the Monoamine Oxidase (MAO) enzyme, including Isocarboxazid, Phenelzine, Toloxatone, and Tranylcypromine.
Contraindications
Metalone Plus is contraindicated in cases of infectious conjunctivitis or keratitis, corneal injuries or ulcers, glaucoma, conditions involving stromal damage to the cornea or sclera, and dry eyes.
Side Effects
Possible side effects include mild burning sensations, reversible increases in intraocular pressure, reactive hyperemia, cataract formation, corneal defects, glaucoma, and systemic effects from prolonged use.
Pregnancy & Lactation
There is limited data on the use of Fluorometholone and Tetrahydrozoline during pregnancy and breastfeeding. Use during pregnancy should only occur if the benefits significantly outweigh any potential risks to the fetus.
Precautions & Warnings
Avoid extended use of Metalone Plus. It should be prescribed cautiously in patients with severe circulatory issues, metabolic disorders, those on MAOIs or other potentially hypertensive medications, and those with a history of cataracts or herpes simplex infections.
Overdose Effects
Under normal usage, the likelihood of overdose is minimal. There is insufficient data on fluorometholone overdoses, but acute symptoms of Tetrahydrozoline overdose may include CNS, cardiac, and psychiatric disturbances, mydriasis, cyanosis, and fever.
In cases of accidental ingestion and symptom onset, possible interventions include administering activated charcoal, gastric lavage, and oxygen ventilation. Phentolamine may be used intravenously to manage hypertension; however, vasopressors should be avoided. Antipyretics and anticonvulsants can be provided as needed.

Storage Conditions
Store out of reach of children in a cool, dry place, away from heat and direct light. Discard any unused drops more than 4 weeks after opening.
