Indications
Luraprex, an atypical antipsychotic, is prescribed for various conditions including:
- Schizophrenia
- Depressive episodes associated with Bipolar I Disorder (bipolar depression), either as monotherapy or in combination with lithium or valproate.
Pharmacology
The effectiveness of Lurasidone in treating schizophrenia may be attributed to its dual action of blocking central Dopamine D2 and Serotonin 5HT2A receptors.
Dosage & Administration
For Schizophrenia:
- Starting Dose: 40 mg once daily
- Recommended Dose: 40 mg to 160 mg once daily
For Bipolar Depression:
- Starting Dose: 20 mg once daily
- Recommended Dose: 20 mg to 120 mg once daily
It’s advisable to take Lurasidone with food to enhance its absorption. Always follow your healthcare provider’s guidance.
Interactions
When taken with moderate inhibitors of CYP3A4 (such as Diltiazem, Atazanavir, Erythromycin, Fluconazole, Verapamil, etc.), the dose of Luraprex should be halved. Avoid consuming grapefruit or grapefruit juice while on Luraprex treatment, as it may affect its concentration.
Contraindications
Avoid Luraprex if you have hypersensitivity to its components, or if you are concurrently using strong CYP3A4 inhibitors (like Ketoconazole) or inducers (like Rifampin). It’s also contraindicated in dementia-related psychosis.
Side Effects
Common side effects of Luraprex include somnolence, akathisia, extrapyramidal symptoms, and nausea.
Pregnancy & Lactation
Lurasidone falls under Pregnancy Category B. Its use during pregnancy should be determined by weighing the potential benefits against the risks. Avoid breastfeeding while on Lurasidone.
Precautions & Warnings
Some precautions include increased risk of cerebrovascular adverse reactions in elderly patients with dementia-related psychosis, neuroleptic malignant syndrome, tardive dyskinesia, metabolic changes, hyperglycemia, dyslipidemia, weight gain, hyperprolactinemia, leukopenia, neutropenia, agranulocytosis, orthostatic hypotension, and syncope.
Use in Special Populations
For patients with moderate and severe renal impairment, adjust the starting and maximum doses accordingly. Similarly, in cases of moderate and severe hepatic impairment, dosage adjustments are necessary.
