Indications

Lozana is prescribed for:

• Endometriosis
• Benign breast disease
• Menorrhagia
• Gynecomastia
• Preoperative endometrial thinning before hysteroscopic procedures

Pharmacology

Lozana, a gonadotropin inhibitor, works by suppressing the pituitary-ovarian axis and inhibiting the secretion of pituitary gonadotropins. This action reduces ovarian estrogen production, leading to atrophy of both normal and ectopic endometrial tissues, thus facilitating the treatment of endometriosis. While the exact mechanism in fibrocystic breast disease is unclear, it likely involves decreased estrogen stimulation.

Dosage & Administration

• Endometriosis: 200-800 mg daily in up to 4 divided doses for 6 months (up to 9 months if needed).
• Menorrhagia: 100-200 mg daily, with 200 mg for 3 months typically effective.
• Severe cyclical mastalgia: 100-400 mg daily for 3-6 months.
• Benign breast cysts: 300 mg daily for 3-6 months.
• Gynecomastia: 400 mg daily in divided doses for 6 months (200 mg for adolescents).
• Preoperative thinning: 400-800 mg daily for 3-6 weeks.

Interactions

Lozana may increase insulin resistance, interact with estrogens and progestogens, and affect the plasma levels of anticonvulsants and antihypertensive medications. Caution is advised for patients on migraine medications, as Lozana may provoke migraines.

Contraindications

• Pregnancy and breastfeeding
• Impaired liver, kidney, or heart function
• Porphyria
• Thromboembolic disorders
• Known hypersensitivity to danazol

Side Effects

Common side effects may include nausea, dizziness, skin reactions, mood changes, and weight gain. More severe effects can involve thrombotic events and alterations in lipoproteins.

Pregnancy & Lactation

Lozana should not be used during pregnancy due to potential risks, including virilization of female fetuses. Nursing mothers should avoid Lozana.

Precautions & Warnings

Use with caution in patients with cardiovascular or hepatic issues, diabetes, and a history of thrombosis. Discontinue use if signs of virilization appear.
