Indications
This solution is used to manage hypovolemia resulting from surgery, hemorrhage, trauma, excessive sweating, severe diarrhea, vomiting, or fluid loss due to nephritic conditions. It helps in addressing typical hypovolemic shock by replenishing fluids and electrolytes. It’s also beneficial for severe plasma loss from intestinal obstruction, burns, or significant skin conditions. Primarily, it serves as a fluid and electrolyte replenisher and may be an alternative to Sodium Bicarbonate in treating metabolic acidosis linked to dehydration, as well as for urine alkalinization.
Composition
This sterile solution contains Sodium Chloride, Potassium Chloride, Calcium Chloride, and Sodium Lactate. Each 100ml includes:
- Sodium Chloride BP: 0.6 g
- Potassium Chloride BP: 0.04 g
- Calcium Chloride BP: 0.027 g
- Sodium Lactate 50% solution USP: equivalent to 0.32 g of Sodium Lactate
Each litre contains the following electrolytes:
- Sodium: 131 mmol
- Potassium: 5 mmol
- Calcium: 2 mmol
- Bicarbonate (as lactate): 29 mmol
- Chloride: 111 mmol
Dosage
The infusion volume and rate depend on the patient’s needs, guided by the physician’s judgment. It varies with age, weight, and clinical condition. A typical recommended flow rate is up to 100 drops per minute for a 70 kg adult. For burn patients, the Parkland formula is used: 4 ml/kg per percentage of Body Surface Area (BSA) burned (e.g., a person weighing 60 kg with 30% BSA burns would require 7,200 ml in 24 hours—half in the first 8 hours, and the remainder over 16 hours).

Administration
- Inspect the infusion set and solution before use.
- Gently pull off the protective cover of the Eurohead.
- Hold the Eurohead lightly without squeezing the bag.
- Fully open the flow regulator and grip the top of the giving set, avoiding the membrane vent.
- Insert the administration set spike into the Eurohead and connect it securely to the needle.
- Allow the fluid to flow to the needle tip, then close.
- Remove the needle cover and insert the needle after cleaning the injection site with antiseptic.
- Secure the puncture site with tape.
- Start the infusion, adjusting the drip speed as necessary.
Contraindications
Avoid using this solution if you have diabetes mellitus, as one of the lactate isomers may promote glucose production. Consult your doctor before use.
Side Effects
Mild swelling in the hands, ankles, or feet due to fluid retention can occur. Rarely, fluid retention may affect the lungs, causing breathing difficulty. Other potential side effects include nausea, vomiting, headache, dizziness, drowsiness, confusion, and localized inflammation around the injection site.
Pregnancy & Lactation
Classified as Pregnancy Category C, the effects of this solution on fetal development are unknown. It should only be used during pregnancy if deemed absolutely necessary by a healthcare provider.
Precautions & Warnings
Lactoride should not be administered rapidly or over prolonged periods. As the solution contains multiple electrolytes, caution is required in patients at risk of electrolyte imbalance, including those who are pregnant, have renal impairment, heart failure, or pulmonary congestion, or are taking potassium-sparing diuretics. Discontinue use if any adverse reactions occur, and avoid using the solution if it is cloudy, contains particles, or is expired.
