Description

Indications

Kilbac is prescribed for treating infections caused by bacteria sensitive to Cefuroxime, including:

  • Pharyngitis/Tonsillitis due to Streptococcus pyogenes.
  • Acute Bacterial Otitis Media from Streptococcus pneumoniae, Haemophilus influenzae, Moraxella catarrhalis (beta-lactamase producing strains), or Streptococcus pyogenes.
  • Acute Bacterial Maxillary Sinusitis caused by Streptococcus pneumoniae or Haemophilus influenzae (non beta-lactamase producing strains).
  • Lower Respiratory Tract Infections including pneumonia from Streptococcus pneumoniae, Haemophilus influenzae, Klebsiella spp., Staphylococcus aureus, Streptococcus pyogenes, and E. coli.
  • Acute Bacterial Exacerbation of Chronic Bronchitis from Streptococcus pneumoniae, Haemophilus influenzae, or Haemophilus parainfluenzae (beta-lactamase negative strains).
  • Skin and Skin-Structure Infections from Staphylococcus aureus or Streptococcus pyogenes.
  • Urinary Tract Infections caused by E. coli or Klebsiella pneumoniae.
  • Bone and Joint Infections from Staphylococcus aureus.
  • Gonorrhoea from both penicillinase-producing and non-penicillinase strains of Neisseria gonorrhoeae.
  • Early Lyme Disease (Erythema Migrans) caused by Borrelia burgdorferi.

Pharmacology

Kilbac, containing Cefuroxime, is an effective broad-spectrum antibiotic with bactericidal properties against many pathogens, including strains resistant to ampicillin and amoxicillin. Its stability against bacterial beta-lactamase enhances its efficacy in treating infections.

Dosage

For Tablets or Suspension:

  • Adults (13 years and older):
    • Pharyngitis/Tonsillitis: 250 mg b.i.d. for 5-10 days.
    • Acute Sinusitis: 250 mg b.i.d. for 10 days.
    • Chronic Bronchitis Exacerbation: 250-500 mg b.i.d. for 10 days.
    • Uncomplicated Skin Infections: 250-500 mg b.i.d. for 10 days.
    • Uncomplicated UTI: 250 mg b.i.d. for 7-10 days.
    • Gonorrhoea: 1000 mg as a single dose.
    • Pneumonia: 250-500 mg b.i.d. for 5-10 days.
    • Early Lyme Disease: 500 mg b.i.d. for 20 days.
  • Pediatric Patients (3 months to 12 years):
    • Pharyngitis/Tonsillitis: 20 mg/kg/day b.i.d. for 5-10 days.
    • Acute Otitis Media: 30 mg/kg/day b.i.d. for 10 days.

For Parenteral Use:

  • Adults: 750 mg three times daily, with potential increases for severe infections.
  • Children (3 months and older): 30-100 mg/kg/day, divided into 3 or 4 doses.
  • Surgical Prophylaxis: 1.5 gm IV at anesthesia induction.

Administration

Use freshly reconstituted solutions, which remain potent for at least 24 hours at room temperature or 48 hours at 5°C.

Interaction

No significant hazardous interactions have been reported with Kilbac.

Contraindications

Kilbac is contraindicated for patients with known allergies to cephalosporins.

Side Effects

Kilbac is generally well-tolerated; potential mild side effects include rashes and gastrointestinal disturbances. Prolonged use may lead to overgrowth of non-susceptible organisms like Candida.

Pregnancy & Lactation

Cefuroxime falls under US FDA pregnancy category B. It should only be used during pregnancy if necessary, as it can be excreted in breast milk.

Precautions & Warnings

Administer with caution to patients on potent diuretics or with a history of colitis. Cross-reactions may occur in hypersensitive patients.

Reconstitution

For injections, reconstitute with sterile water as indicated, ensuring proper dispersion before administration.
