Pharmacology
Glucosamine: Glucosamine (2-amino-2-deoxy-alpha-D-glucose) is a naturally occurring amino sugar produced by the body and present in certain foods. It is a crucial building block for synthesizing glycosaminoglycans (GAGs) such as hyaluronic acid, keratan sulfate, and chondroitin sulfate. These GAGs bind with proteins to form proteoglycans, which are essential for maintaining articular cartilage. When cartilage deteriorates, osteoarthritis can develop. Glucosamine also contributes to the formation of ligaments, tendons, nails, and other connective tissues. Supplementing with synthesized glucosamine sulfate increases its levels in the body, promoting cartilage production and repair. It also stimulates chondrocytes in the cartilage, enhancing the production of GAGs and proteoglycans.
Chondroitin Sulfate: This glycosaminoglycan is found in connective tissues, especially articular cartilage. Chondroitin sulfate acts similarly to glucosamine sulfate by providing a substrate for proteoglycan formation. Additionally, it helps protect healthy cartilage from premature breakdown by inhibiting the matrix metalloproteinase (MMP) enzyme that degrades proteoglycans.
The combination of glucosamine and chondroitin sulfate produces a synergistic effect. Research supports that this duo is particularly effective in severe osteoarthritis cases, addressing both symptoms and disease progression. It also prevents osteoarthritis in healthy adults and offers efficacy comparable to NSAIDs, with significantly better tolerability. Furthermore, it aids in the recovery phase of musculoskeletal soft tissue injuries, such as tendon and ligament strains.
Dosage & Administration
- 250/200 mg tablet: 1 to 2 tablets three times daily. Dosage may be adjusted based on individual response and body weight. After 60 days, doses can be tapered as needed. Recommended dosages based on body weight are:
- Under 54 kg: 1000 mg glucosamine sulfate & 800 mg chondroitin sulfate daily
- 54 kg to 91 kg: 1500 mg glucosamine sulfate & 1200 mg chondroitin sulfate daily
- Over 91 kg: 2000 mg glucosamine sulfate & 1600 mg chondroitin sulfate daily
- 750/600 mg tablet: 1 tablet twice daily or as directed by a physician.

Interactions
There are no significant drug interactions reported between glucosamine and chondroitin with antibiotics, antidepressants, antihypertensives, nitrates, antiarrhythmics, anxiolytics, hypoglycemic agents, antisecretives, or antiasthmatics. However, chondroitin may enhance the anticoagulant effects of medications like warfarin and heparin.
Contraindications
While there are no general contraindications for glucosamine and chondroitin, individuals with proven hypersensitivity (e.g., allergies to shellfish or sulfur) should avoid these supplements.
Side Effects
Safety studies indicate that glucosamine sulfate and chondroitin sulfate are well tolerated, with no serious adverse effects reported. Rare occurrences of mild and reversible intestinal flatulence are comparable to placebo effects.
Pregnancy & Lactation
Women who are pregnant or may become pregnant should avoid glucosamine sulfate and chondroitin sulfate supplements, as their effects on fetal development have not been adequately studied. Medical advice is recommended during pregnancy and lactation.
Precautions & Warnings
Diabetics should monitor their blood glucose levels regularly while taking glucosamine. No specific studies have been conducted in patients with renal or hepatic insufficiency; however, the toxicological and pharmacokinetic profiles do not indicate limitations for these patients. Administration to those with severe hepatic or renal impairment should occur under medical supervision. Children should not use glucosamine and chondroitin supplements.
