Indications
Jardimet is prescribed for adults with type 2 diabetes mellitus to supplement diet and exercise in the following cases:
- When Metformin alone does not provide sufficient control at maximum tolerated doses.
- In combination with other diabetes medications when Metformin and these medications fail to control blood sugar.
- For patients already using separate Empagliflozin and Metformin tablets.
Pharmacology
Empagliflozin inhibits Sodium-Glucose Co-Transporter 2 (SGLT2), reducing glucose reabsorption by the kidneys. This increases urinary glucose excretion, lowering renal glucose threshold and aiding in glycemic control. Metformin Hydrochloride, a biguanide, further improves insulin sensitivity, reduces hepatic glucose production, and enhances peripheral glucose uptake.
Dosage & Administration
Dosage is personalized based on effectiveness and tolerance. Take Jardimet twice daily with meals. Begin with a gradual dose escalation to manage potential gastrointestinal effects of Metformin. Maximum daily doses are 2000 mg of Metformin Hydrochloride and 25 mg of Empagliflozin.
Interactions
- Co-administration with diuretics can increase urine volume.
- Risk of hypoglycemia increases when Jardimet is used with insulin or insulin secretagogues.
- Monitoring glucose via urine tests is not recommended due to increased urinary glucose excretion with SGLT2 inhibitors.
- Drugs affecting Metformin clearance or increasing the risk of lactic acidosis should be used cautiously.
Contraindications
Avoid Jardimet in cases of hypersensitivity to Empagliflozin or Metformin, acute metabolic acidosis, severe renal failure (GFR <30 ml/min), and conditions predisposing to renal function impairment.
Side Effects
Common adverse reactions include gastrointestinal symptoms and urinary tract infections. Hypoglycemia and genital mycotic infections are also reported. Severe adverse reactions such as lactic acidosis are rare but require immediate medical attention.
Pregnancy & Lactation
Jardimet is not recommended during pregnancy or breastfeeding due to potential risks to the fetus and lack of data on excretion into breast milk.
Precautions & Warnings
Monitor for lactic acidosis, especially in cases of renal impairment or conditions predisposing to hypotension. Regularly assess renal function and adjust therapy accordingly.
Overdose Effects
In case of overdose, manage clinically and consider hemodialysis for Metformin removal, although Empagliflozin removal via hemodialysis is not established.
