Indications
Intobac is a topical antibiotic specifically designed for treating external bacterial infections of the eye caused by susceptible pathogens. It is effective against:
Gram-positive Bacteria:
- Staphylococcus aureus
- Staphylococcus epidermidis
- Streptococcus pneumoniae
- Group A beta-hemolytic Streptococcus spp. and some non-hemolytic species.
Gram-negative Bacteria:
- Escherichia coli
- Pseudomonas aeruginosa
- Enterobacter aerogenes
- Klebsiella spp.
- Proteus mirabilis
- Proteus vulgaris
- Haemophilus influenzae
- Morganella morganii
- Acinetobacter calcoaceticus
- Providencia
- Serratia

Pharmacology
Intobac contains Tobramycin, an aminoglycoside antibiotic that exhibits bactericidal properties by penetrating sensitive bacterial cells through an active transport mechanism. Inside the cell, Tobramycin binds to the 30S and, to a lesser extent, the 50S subunits of the bacterial ribosome. This binding disrupts protein synthesis and can lead to errors in genetic transcription. While the exact mechanism of cell death remains partially understood, it may also involve alterations in membrane permeability.
Dosage & Administration
Ophthalmic Ointment:
- For mild to moderate infections, apply a small amount 2-3 times daily in the conjunctival sac(s).
- For severe infections, apply 3-4 times daily until improvement is noted, then gradually reduce the dosage.
Ophthalmic Solution:
- For mild to moderate infections, instill 1-2 drops into the affected eye(s) every 4 hours.
- For severe infections, instill 2 drops into the affected eye(s) hourly until improvement is observed.
Pediatric Use:
Safety and efficacy in children under 1 year of age have not been established.

Interactions
Caution is advised when administering Intobac to patients taking neuromuscular blocking agents or ototoxic medications.
Contraindications
