Intobac Eye Drop

Indications

Intobac is a topical antibiotic specifically designed for treating external bacterial infections of the eye caused by susceptible pathogens. It is effective against:

Gram-positive Bacteria:

• Staphylococcus aureus
• Staphylococcus epidermidis
• Streptococcus pneumoniae
• Group A beta-hemolytic Streptococcus spp. and some non-hemolytic species.

Gram-negative Bacteria:

• Escherichia coli
• Pseudomonas aeruginosa
• Enterobacter aerogenes
• Klebsiella spp.
• Proteus mirabilis
• Proteus vulgaris
• Haemophilus influenzae
• Morganella morganii
• Acinetobacter calcoaceticus
• Providencia
• Serratia
• Salmonella spp. and some strains of Neisseria.

Pharmacology

Intobac contains Tobramycin, an aminoglycoside antibiotic that exhibits bactericidal properties by penetrating sensitive bacterial cells through an active transport mechanism. Inside the cell, Tobramycin binds to the 30S and, to a lesser extent, the 50S subunits of the bacterial ribosome. This binding disrupts protein synthesis and can lead to errors in genetic transcription. While the exact mechanism of cell death remains partially understood, it may also involve alterations in membrane permeability.

Dosage & Administration

Ophthalmic Ointment:

• For mild to moderate infections, apply a small amount 2-3 times daily in the conjunctival sac(s).
• For severe infections, apply 3-4 times daily until improvement is noted, then gradually reduce the dosage.

Ophthalmic Solution:

• For mild to moderate infections, instill 1-2 drops into the affected eye(s) every 4 hours.
• For severe infections, instill 2 drops into the affected eye(s) hourly until improvement is observed.

Pediatric Use:
Safety and efficacy in children under 1 year of age have not been established.

Interactions

Caution is advised when administering Intobac to patients taking neuromuscular blocking agents or ototoxic medications.

Contraindications

Intobac is contraindicated in individuals who are hypersensitive to Tobramycin or any components of the formulation.

Side Effects

Common side effects of Intobac ophthalmic solution may include localized ocular toxicity, conjunctival redness, and hypersensitivity reactions such as eyelid itching and swelling.

Pregnancy & Lactation

Intobac falls under Pregnancy Category B. There are no well-controlled studies in pregnant women, so it should only be used if the potential benefits justify any possible risks to the fetus. This medication may be excreted in human milk; thus, a decision should be made regarding nursing during treatment.

Precautions & Warnings

Some patients may experience mild sensitivity to topically applied aminoglycosides. If any sensitivity reactions occur, discontinue use. Extended use may lead to the overgrowth of non-susceptible organisms and fungi.

Overdose Effects

Signs and symptoms of overdose may mirror the side effects previously mentioned.

Storage Conditions

Store in a cool, dry place away from light (below 25°C). Keep out of reach of children. Use within 4 weeks of opening.
