Description

Indications

  • Glipita is prescribed as an adjunct to diet and exercise to enhance glycemic control in adults with type 2 diabetes mellitus.
  • Important Note: Glipita is not suitable for individuals with type 1 diabetes or diabetic ketoacidosis and has not been evaluated for patients with a history of pancreatitis.

Pharmacology

Glipita contains Sitagliptin, a DPP-4 inhibitor. It enhances the action of incretin hormones by preventing their breakdown, leading to increased insulin release and reduced glucagon levels, thereby improving blood sugar control.

Dosage & Administration

  • Adults & Adolescents (12+ years): 50 mg twice daily or 100 mg once daily, with or without food.
  • Children (2-11 years):
    • Weighing 25 kg or more: 5 ml of oral solution daily.
    • Weighing 10-24 kg: 2.5 ml of oral solution daily.

Interactions

  • Drug Interactions: Monitor when used with ketoconazole, erythromycin, or CYP3A4 inhibitors. No major interactions with metformin, glyburide, or other common drugs.
  • Food Interactions: Avoid grapefruit and grapefruit juice.

Contraindications

  • Hypersensitivity to Sitagliptin or any excipients.

Side Effects

  • Common: Headache, upper respiratory infections, nasopharyngitis.
  • Rare: Palpitations, tachycardia.

Pregnancy & Lactation

  • Pregnancy Category B: Use only if necessary. Caution is advised during lactation as the drug may be excreted in milk.

Precautions & Warnings

  • Pancreatitis: Discontinue if pancreatitis is suspected.
  • Renal Insufficiency: Adjust dosage based on renal function.
  • Hypoglycemia Risk: May require dose adjustment when used with sulfonylureas or insulin.

Special Populations

  • Elderly: Use with caution due to potential renal function decline.
  • Pediatric: Safety and effectiveness not established for those under 18.
  • Renal Insufficiency: Dosage adjustments required for moderate to severe cases.

Overdose Effects

  • Symptoms: Mostly mild, such as somnolence. Supportive care recommended. Glipita is modestly dialyzable.
