Indications
Gastalfet is prescribed for the treatment and prevention of:
- Duodenal ulcers
- Gastric ulcers
- Chronic gastritis
- Prophylaxis of gastrointestinal bleeding from stress ulcers in critically ill patients
Pharmacology
Gastalfet contains Sucralfate, a non-systemic agent minimally absorbed from the gastrointestinal tract. It forms a protective barrier at ulcer sites, aiding in the healing of gastric and duodenal ulcers. Additionally, Sucralfate inhibits pepsin and bile activity, enhancing ulcer protection.
Dosage
- For Duodenal Ulcers, Gastric Ulcers, and Chronic Gastritis:
- Adults: 2 gm twice daily on rising and at bedtime, or 1 gm four times a day, one hour before meals and at bedtime. Maximum dose is 8 gm daily, up to twelve weeks if necessary.
- Pediatric Population: Safety and efficacy in children under 14 years are not established.
- Elderly: No special dosage adjustments are required, but use the lowest effective dose.
- For Prophylaxis of Stress Ulceration:
- Adults: 1 gm orally or via a nasogastric tube 4 to 6 times daily. To prevent tube clogging, flush with 10 ml of water after each dose. Duration of use should be individualized but generally not exceed 14 days.
Note: Follow your healthcare provider’s instructions.
Administration
Take Gastalfet on an empty stomach. Avoid administering antacids within 30 minutes of taking Gastalfet.
Interactions
Gastalfet may reduce the absorption of various drugs such as Fluoroquinolones, Tetracycline, and Ketoconazole. To mitigate this, separate the administration of these drugs by at least two hours. Avoid co-administration with citrate preparations due to potential increased aluminum absorption.
Contraindications
Do not use if hypersensitive to Sucralfate.
Side Effects
Common side effects include headache, nausea, abdominal pain, constipation, diarrhea, and urticaria. Rarely, bezoar formation and hyperglycemia in diabetic patients may occur.
Pregnancy & Lactation
Use during pregnancy only if clearly necessary. It is unknown if Sucralfate is excreted in breast milk, so exercise caution when administering to breastfeeding women.
Precautions & Warnings
Use Gastalfet cautiously in patients with renal impairment due to the risk of aluminum accumulation. It is not recommended for individuals on dialysis or with severe renal conditions. Monitor renal function and avoid other aluminum-containing medications.
Use in Special Populations
- Pediatric Population: Not recommended for children under 14 years.
- Elderly: No special adjustments needed.
- Renal Impairment: Use with caution in patients with renal insufficiency.
- Driving & Operating Machinery: Avoid driving if experiencing dizziness or drowsiness.
