Indications
Fosfomycin is primarily prescribed for the treatment of uncomplicated urinary tract infections (UTIs), specifically acute cystitis in women, caused by susceptible strains of Escherichia coli and Enterococcus faecalis.

Pharmacology
Fosfomycin is effective against a wide range of gram-positive and gram-negative aerobic bacteria, which are commonly responsible for uncomplicated UTIs. Fosfomycin Trometamol is a phosphonic acid derivative, designed for oral use. This broad-spectrum, bactericidal antibiotic works by inhibiting the enzyme enolpyruvyl transferase, which disrupts the synthesis of bacterial cell walls. By blocking the formation of key bacterial cell wall components, Fosfomycin effectively kills bacteria.
Dosage & Administration
For women 18 years and older suffering from acute cystitis, the recommended dosage is one sachet of Fosfomycin, which can be taken with or without food. This medicine is not suitable for use in children.
Preparation:
- Pour 100 ml of purified water into a glass.
- Add the entire contents of one Fosfomycin sachet and stir well until completely dissolved.
- Consume the mixture immediately after preparation.

Drug Interactions
Fosfomycin may interact with metoclopramide, a drug that increases gastrointestinal motility. This interaction can lower the serum concentration and urinary excretion of Fosfomycin. Similar effects may be seen with other medications that enhance gastrointestinal motility.
Contraindications
Fosfomycin should not be used by individuals who are hypersensitive to the drug, those with severe renal insufficiency, or patients undergoing hemodialysis.

Common side effects reported during clinical trials (affecting more than 1% of users) include:
- Diarrhea: 10.4%
- Headache: 10.3%
- Vaginitis: 7.6%
- Nausea: 5.2%
- Rhinitis: 4.5%
- Back pain: 3.0%
- Dysmenorrhea: 2.6%
Other less common side effects (occurring in less than 1% of users) include abnormal stools, anorexia, dry mouth, dysuria, ear disorders, fever, and others. Contact your healthcare provider if you experience persistent side effects.
Pregnancy & Lactation
Fosfomycin falls under Pregnancy Category B. It should only be used during pregnancy if the benefits outweigh the potential risks. If breastfeeding, consider whether to discontinue the drug or stop nursing, based on the importance of the medication to the mother.
Precautions & Warnings
Use of Fosfomycin can lead to Clostridium difficile-associated diarrhea (CDAD), a serious condition caused by an overgrowth of Clostridium difficile bacteria in the colon. Avoid exceeding the recommended single dose for treating acute cystitis. Repeated daily doses have shown no additional benefit in improving clinical outcomes and may increase the risk of side effects.
Overdose Effects
Taking an overdose of Fosfomycin may intensify side effects. If you suspect an overdose, seek medical advice immediately.
