Indications
Fonidel is a Factor Xa inhibitor (anticoagulant) used for prophylaxis of deep vein thrombosis (DVT) in various surgical procedures and for the treatment of DVT or acute pulmonary embolism (PE) in combination with warfarin.
Pharmacology
Fonidel selectively inhibits Factor Xa, interrupting the blood coagulation cascade and inhibiting thrombus development without affecting platelet function or fibrinolytic activity.
Dosage & Administration
The dosage varies based on the indication. For prophylaxis, it’s typically administered once daily after surgery, while for treatment, the dosage is adjusted according to body weight and is continued until a target INR is achieved with warfarin.
Interaction
Co-administration with agents that increase bleeding risk should be avoided unless essential, and patients should be closely monitored if such co-administration is necessary.
Contraindications
Fonidel is contraindicated in severe renal impairment, active major bleeding, bacterial endocarditis, thrombocytopenia, low body weight (<50 kg), and hypersensitivity to Fondaparinux Sodium.
Side Effects
Common adverse reactions include bleeding complications, injection site irritation, rash, anemia, insomnia, dizziness, hypotension, and confusion.
Pregnancy & Lactation
It is classified as Pregnancy Category B, and caution is advised during breastfeeding due to unknown excretion into human milk.
Precautions & Warnings
Caution is recommended in patients with increased bleeding risk, renal impairment, low body weight, or undergoing neuraxial intervention. Regular monitoring of blood counts and serum creatinine levels is advised.
