Pharmacology
Fluphenazine acts by blocking postsynaptic dopamine D1 and D2 receptors within the mesolimbic system, leading to a decrease in the release of hormones from the hypothalamus and pituitary gland. This results in a calming effect, reduction of aggressiveness, and alleviation of hallucinations and delusions. Fluphenazine Decanoate, a phenothiazine derivative, is recognized for its high potency as an antipsychotic agent and features a notably extended duration of action, administered through deep intramuscular injection. The esterification of fluphenazine with decanoic acid significantly prolongs its effects without diminishing its efficacy.
Dosage & Administration
Fluphenazine Decanoate should be administered via deep intramuscular injection into the gluteal muscle. The initial test dose is 12.5 mg (6.25 mg for elderly patients). After 4-7 days, doses of 12.5-100 mg may be repeated at intervals of 14-35 days, adjusted according to individual response. This medication is not recommended for children.

Interaction
Fenazine may counteract the antihypertensive effects of medications such as Guanethidine, Methyldopa, and Clonidine. It can also increase lithium toxicity when used concurrently. Additionally, antacids may reduce the bioavailability of phenothiazines. Combining Fenazine with alcohol, barbiturates, hypnotics, sedatives, opiates, and antihistamines can produce additive CNS depressant effects.
Contraindications
Fluphenazine Decanoate is contraindicated in individuals with hypersensitivity to the drug. It is also not suitable for patients who are comatose or severely depressed, have blood disorders, liver disease, or are in the third trimester of pregnancy or breastfeeding.
Side Effects
Common side effects include tardive dyskinesia, sedation, confusion, hypotension, and hyperprolactinemia, which may lead to galactorrhea and amenorrhea in women. In males, it can cause loss of libido, impotence, and sterility. Additional potential adverse reactions encompass allergic reactions, cholestatic jaundice, deposits in cornea and lens, skin pigmentation changes, agranulocytosis, and neuroleptic malignant syndrome.
Pregnancy & Lactation
The safety of Fluphenazine Decanoate during pregnancy has not been fully established. Therefore, healthcare providers should carefully weigh the potential risks against benefits before prescribing this medication to pregnant women.
Precautions & Warnings
Special precautions should be taken in patients with a history of convulsive disorders, liver, kidney, cerebrovascular, respiratory, and cardiovascular diseases. Careful monitoring is also recommended for those with pheochromocytoma, bone marrow suppression, and in elderly or weakened patients.
Overdose Effects
Symptoms of overdose may include muscle weakness, irregular heartbeat, and difficulty breathing.
