Indications
Exovate N is designed for effective management of various skin conditions in adults, the elderly, and children over 1 year. It is especially beneficial for:
- Persistent eczemas and neurodermatoses
- Psoriasis (excluding extensive plaque psoriasis) with suspected or present bacterial or fungal infections
- Inflammatory skin conditions resistant to less potent steroids
Pharmacology
Exovate N combines Clobetasol Propionate, a potent corticosteroid known for its strong anti-inflammatory effects, with Neomycin Sulphate, an aminoglycoside antibiotic, and Nystatin, a powerful antifungal. Clobetasol Propionate reduces inflammation through vasoconstriction and decreased collagen production. Neomycin inhibits bacterial protein synthesis by binding to ribosomal sub-units, while Nystatin disrupts fungal cell membranes to kill fungi and yeasts.
Dosage & Administration
- Adults: Apply a thin layer to the affected area once or twice daily. For resistant lesions, occlude with polythene overnight if needed. Do not use for more than 7 days without medical supervision. If treatment exceeds 4 weeks, review the patient’s condition regularly.
- Elderly: Safe for use with caution, especially if renal function is impaired.
- Children: Suitable for children over 2 years; not recommended for neonates and infants due to potential increased absorption.
Interaction
Systemic absorption of Neomycin Sulphate is minimal with proper use, so significant drug interactions are unlikely. No known hazardous interactions with Clobetasol Propionate or Nystatin.
Contraindications
Not suitable for individuals with rosacea, acne vulgaris, perioral dermatitis, primary cutaneous viral infections (e.g., herpes simplex), or hypersensitivity to any ingredient.
Side Effects
Potential side effects include systemic absorption leading to hypercortisolism, especially with prolonged or extensive use. Local skin changes like thinning, striae, and pigmentation changes can occur. The risk is higher in infants and children and with occlusive dressings.
Pregnancy & Lactation
Limited data on the effects of Neomycin and Clobetasol Propionate during pregnancy and lactation. Use only if absolutely necessary and under medical supervision, as Neomycin can cross the placenta and potentially affect the fetus.
Precautions & Warnings
Avoid long-term use, especially in children and on facial areas, due to the risk of adrenal suppression. Prevent contact with the eyes to avoid potential glaucoma. If accidental eye contact occurs, rinse thoroughly with water.
Overdose Effects
Acute overdose is unlikely. In cases of chronic misuse, symptoms of hypercortisolism may appear, requiring gradual discontinuation of the topical steroid.
