Indications

Our medication, Epitra, serves multiple purposes. It’s prescribed primarily for panic disorder treatment, whether with or without agoraphobia. This disorder is characterized by sudden, unexpected panic attacks and the accompanying worry about experiencing more attacks or their consequences.

Epitra is also indicated for Lennox-Gastaut Syndrome, particularly the petit mal variant, as well as for akinetic and myoclonic seizures. It may also be considered for patients with absence seizures who haven’t responded to other treatments.

While short-term effectiveness is well-established, it’s important to note that the long-term use of Epitra beyond 9 weeks hasn’t been extensively studied in controlled clinical trials. Physicians should periodically reassess its long-term benefits for individual patients.

Pharmacology

Clonazepam, the active ingredient in Epitra, shares common pharmacological traits with benzodiazepines, including anticonvulsant, sedative, muscle relaxant, and anxiolytic effects. Its central actions involve enhancing GABAergic neurotransmission at inhibitory synapses. This modulation increases the action of released GABA, resulting in heightened postsynaptic transmembrane chloride ion flux.

Additionally, animal studies suggest clonazepam’s impact on serotonin levels. It has shown rapid suppression of various paroxysmal activities, including certain seizure types, both in animals and humans.

Dosage & Administration

For adults with seizure disorders, the initial daily dose should not exceed 1.5 mg, divided into three doses. Dosage adjustments can be made every 3 days, based on response and tolerance, up to a maximum daily dose of 20 mg. For panic disorder, adults typically start with 0.25 mg twice daily, with a gradual increase to 1 mg/day.

Pediatric dosages are weight-based to minimize drowsiness, with careful attention to dose adjustments for infants and children.

Interaction

Epitra’s interaction with other drugs hasn’t been extensively studied, although it doesn’t appear to affect the pharmacokinetics of certain anticonvulsants.

Contraindications

Epitra should be avoided in patients with a history of benzodiazepine hypersensitivity or significant liver disease. It’s contraindicated in acute narrow-angle glaucoma.
