Indications
Epilep CR is prescribed for the management of:
- Partial and secondary generalized tonic-clonic seizures
- Primary generalized tonic-clonic seizures
- Trigeminal neuralgia
- Prophylaxis of bipolar disorder
Pharmacology
Epilep CR contains carbamazepine, which inhibits excitatory neural activity by reducing the influx of sodium ions across cell membranes. This action decreases neural discharge and synaptic propagation. Carbamazepine also stimulates the release of antidiuretic hormone (ADH), enhancing water reabsorption.
Dosage & Administration
Epilepsy:
- Adults & Children Over 12 Years: Start with 200 mg twice daily (b.i.d.) for tablets or controlled-release tablets, or 1 teaspoon four times daily (q.i.d.) for suspension (400 mg/day). Adjust dosage weekly by up to 200 mg/day until optimal response is achieved. Maintenance doses generally range from 800-1200 mg daily.
- Children 12-15 Years: Dosage should not exceed 1000 mg daily, with up to 1600 mg in rare cases. Maintenance doses typically range from 800-1200 mg daily.
- Children 6-12 Years: Initial dosage is 100 mg b.i.d. for tablets or controlled-release tablets, or 1/2 teaspoon q.i.d. for suspension (200 mg/day). Increase by 100 mg/day weekly until optimal response is achieved. Dosage generally should not exceed 1000 mg daily. Maintenance doses are usually 400-800 mg daily.
- Children Under 6 Years: Start with 10-20 mg/kg/day, administered b.i.d. or t.i.d. as tablets, or q.i.d. as suspension. Adjust weekly for optimal response, with maintenance doses typically below 35 mg/kg daily. Measure plasma levels if response is unsatisfactory.
Combination Therapy: Epilep CR can be used alone or with other anticonvulsants. If added to existing therapy, introduce gradually and adjust other medications as needed.
Trigeminal Neuralgia:
- Initial Dose: Start with 100 mg b.i.d. for tablets or controlled-release tablets, or 1/2 teaspoon q.i.d. for suspension, totaling 200 mg daily. Increase by 100 mg every 12 hours for tablets or 50 mg q.i.d. for suspension as needed, up to 1200 mg daily. Maintenance usually requires 400-800 mg daily.

Interactions
Epilep CR may affect the efficacy of oral contraceptives and interact with hepatic enzyme inducers like phenytoin, potentially requiring dosage adjustments.
Contraindications
Do not use Epilep CR if allergic to any of its ingredients or if you have:
- Atrioventricular block
- Bone marrow depression
- Porphyrias
- Allergy to tricyclic antidepressants
- Recent use of monoamine-oxidase inhibitors (MAOIs)
Side Effects
Common side effects include dizziness, drowsiness, ataxia, dry mouth, abdominal pain, nausea, vomiting, and bradycardia. Severe reactions may affect the hematologic, dermatologic, and cardiovascular systems. Sun sensitivity may occur; use sunscreen or protective clothing.
Pregnancy & Lactation
Epilep CR is classified as Pregnancy Category D. It passes into breast milk. Discuss with your doctor whether to continue nursing or adjust the medication.
Precautions & Warnings
- May cause dizziness or drowsiness; avoid hazardous activities like driving if affected.
- Monitor for skin reactions; report any rash or skin issues to your doctor.
- Be aware of potential liver problems; consult your doctor if symptoms like jaundice or dark urine occur.
- Epilep CR may reduce the effectiveness of hormonal contraceptives; consider using alternative or additional contraceptive methods.
- Do not abruptly stop the medication; taper off gradually under medical supervision if needed.
Caution in: Mixed seizures, elderly individuals, and those with a history of heart, kidney, liver disease, psychosis, or blood disorders.
