Description

Indications

Clovate is indicated for the treatment of various skin conditions in both adults and children over 1 year of age. It is particularly effective in treating:

  • Psoriasis (excluding widespread plaque psoriasis)
  • Recalcitrant dermatoses
  • Lichen planus
  • Discoid lupus erythematosus
  • Other resistant skin conditions that do not respond to milder steroid treatments.

Composition

  • Clobetasol Propionate Cream/Ointment: Each gram contains 0.5 mg of Clobetasol Propionate BP (0.05% w/w).
  • Clobetasol Propionate Scalp Application/Lotion: Each gram contains 0.5 mg of Clobetasol Propionate BP (0.05% w/w).
  • Clobetasol Propionate Shampoo: Each gram contains 0.5 mg of Clobetasol Propionate BP (0.05% w/w).
  • Clobetasol Propionate Spray: Each gram contains 0.5 mg of Clobetasol Propionate BP (0.05% w/w).

Pharmacology

Clobetasol Propionate is a potent topical corticosteroid with powerful anti-inflammatory, antipruritic, and vasoconstrictive effects. It works by:

  • Reducing the activity of mast cells and eosinophils.
  • Suppressing the production of pro-inflammatory cytokines.
  • Inhibiting arachidonic acid metabolism, which reduces the synthesis of key inflammatory mediators (prostaglandins and leukotrienes).

It also has immunosuppressive properties, reducing immune system function by affecting lymphocytes and immunoglobulins.


Dosage & Administration

  • Cream/Ointment: Apply a thin layer to the affected areas twice daily. The maximum weekly dose should not exceed 50 gm/week. For children, limit treatment to 5 days.
  • Spray: Apply to the affected scalp areas 1-2 times daily. The total dosage should not exceed 50 ml per week.
  • Shampoo: Apply to a dry scalp, leave for 15 minutes before rinsing. Do not exceed 50 g per week, and limit use to 4 weeks.

Contraindications

Clovate should not be used in:

  • Patients with rosacea, acne vulgaris, or perioral dermatitis.
  • Active skin infections (bacterial, viral, fungal).
  • Children under one year.

It should also not be used in infected skin conditions, including fungal or bacterial infections like impetigo or candidiasis.


Side Effects

Potential side effects include:

  • Skin thinning or striae with long-term use.
  • Hypercorticism from excessive use, especially on large areas.
  • Possible exacerbation of psoriasis, including pustular outbreaks.
  • Rare cases of pigmentation changes or hypertrichosis.

Discontinue use immediately if hypersensitivity reactions occur.


Pregnancy & Lactation

  • Pregnancy: Use Clobetasol Propionate during pregnancy only if absolutely necessary, as it may cause fetal development issues in animal studies.
  • Lactation: It is unknown if the drug passes into breast milk, so use with caution during breastfeeding.

Precautions & Warnings

  • Avoid prolonged use on the face and in children, as it may cause adrenal suppression.
  • Use caution when applying near the eyes, as it may lead to glaucoma or cataracts.
  • Monitor patients with psoriasis carefully, as topical steroids may worsen the condition.
  • If signs of infection occur, discontinue corticosteroid therapy and begin anti-microbial treatment.

Overdose Effects

Overuse may lead to symptoms of hypercortisolism. Discontinue treatment immediately if overdose symptoms appear.
