Description

Indications

  • Bronchospasm: Brodil Inhaler is recommended for managing or preventing bronchospasm in patients with bronchial asthma. It is also used for the treatment of reversible airway obstruction associated with conditions like bronchitis and emphysema.
  • Exercise-Induced Bronchospasm: Brodil Inhaler can provide relief from acute breathlessness attacks. It is also effective when used as a preventive measure before physical exertion or to avoid exercise-induced asthma symptoms.

Pharmacology

Brodil contains Salbutamol, a selective β2-adrenoceptor agonist. It primarily targets the β2-receptors in bronchial smooth muscles, causing bronchodilation without affecting β1-receptors in cardiac muscles. This provides quick relief (within 5 minutes) and a duration of action of 4-6 hours in reversible airway obstruction. Salbutamol also has anti-inflammatory effects by inhibiting the release of bronchoconstrictor mediators like histamine, prostaglandins, and neutrophil chemotactic factors.

Dosage Instructions

  • For Acute Bronchospasm Relief:
    • Adults: 1 or 2 puffs as needed. Maximum: 8 puffs in 24 hours.
    • Children: Half the adult dose.
  • To Prevent Exercise-Induced Bronchospasm:
    • Adults: 2 puffs 15 minutes before exercise or allergen exposure. Maximum: 2 puffs, up to 4 times per day.
    • Children: Half the adult dose.
  • Chronic Maintenance Therapy:
    • Adults: Up to 200 mcg (2 puffs) four times daily.
    • Children: Up to 200 mcg (2 puffs) four times daily.

Administration Instructions

Using the inhaler correctly is crucial to ensure the medication reaches your lungs. Incorrect usage can reduce its effectiveness. Follow these steps for optimal results:

  1. Remove the inhaler cap.
  2. Shake the inhaler vigorously (at least 6 times) before use.
  3. If the inhaler is new or unused for over a week, test by releasing a puff into the air.
  4. Exhale fully and hold the inhaler upright.
  5. Place the mouthpiece between your teeth, closing lips tightly around it.
  6. Inhale deeply and slowly through your mouth while pressing the canister firmly to release the medication.
  7. Remove the inhaler and hold your breath for at least 10 seconds or as long as comfortable.
  8. If multiple puffs are prescribed, wait 1 minute between inhalations. Shake the inhaler again and repeat steps 4-7.
  9. After use, replace the cap and rinse your mouth with water.

Inhaler Cleaning: Clean your inhaler weekly. Remove the canister and rinse the plastic parts (actuator and cap) with warm water. Never immerse the canister in water. Dry thoroughly and reassemble the inhaler carefully.

Drug Interactions

Brodil Inhaler should not be used with non-selective beta-blockers like propranolol, as it may increase the risk of adverse effects. Concomitant use with certain medications (e.g., xanthines, steroids, diuretics) or in hypoxic conditions may increase the risk of hypokalemia. Regular monitoring of potassium levels is recommended.

Contraindications

Do not use Brodil Inhaler if you have a known hypersensitivity to Salbutamol or any of its components.

Side Effects

While Brodil is generally well tolerated, some side effects may occur, including:

  • Tremors, anxiety, muscle cramps
  • Headaches, palpitations, and tachycardia
  • Mouth and throat irritation

Serious cardiovascular effects like arrhythmias may also be seen in some patients.

Pregnancy & Lactation

Brodil is classified as Pregnancy Category C. There are limited studies on its use during pregnancy, so it should be used only if the benefits outweigh the risks. The effects on breastfeeding are not well understood, so caution is advised.

Warnings and Precautions

  • Paradoxical Bronchospasm: Inhalation may cause paradoxical bronchospasm, which can be life-threatening. Discontinue use immediately if this occurs and seek alternative treatments.
  • Cardiovascular Caution: Brodil Inhaler may cause changes in heart rate or blood pressure. Caution is needed in patients with pre-existing heart conditions like arrhythmias, coronary insufficiency, or hypertension.
  • Hypersensitivity Reactions: Reactions such as urticaria, angioedema, and anaphylaxis have been reported. Discontinue use if any signs of hypersensitivity occur.
  • Other Conditions: Caution is advised in patients with conditions like diabetes, hyperthyroidism, or convulsive disorders. Large doses may aggravate conditions like diabetes or ketoacidosis.

Use in Special Populations

  • Pediatric Use: Brodil Inhaler is suitable for children aged 4 years and older.
