Indications
Bronchospasm
Brodil Inhaler is indicated for managing and preventing bronchospasms associated with bronchial asthma. It is also effective in treating reversible airway obstructions caused by conditions like bronchitis and emphysema.
Exercise-Induced Bronchospasm (EIB)
This inhaler can relieve acute episodes of breathlessness and can also be used preventatively before physical exertion to avert exercise-induced asthma symptoms.
Note: Always use medications as directed by a licensed healthcare professional.
Pharmacology
Salbutamol, a selective β2-adrenoceptor agonist, targets bronchial smooth muscle receptors with minimal effect on cardiac β1-adrenoceptors at therapeutic doses. It provides rapid bronchodilation within 5 minutes, lasting 4–6 hours, to address reversible airway obstruction. Additionally, it exhibits anti-inflammatory properties by inhibiting mast cell mediators such as histamine, prostaglandin D2, and neutrophil chemotactic factors.
Dosage and Administration
Route of Administration:
Use Brodil Inhaler via oral inhalation. Shake the inhaler thoroughly before each use.
For acute bronchospasm relief:
- Adults: 1–2 puffs as required, with a maximum of 8 puffs in 24 hours.
- Children: Half the adult dosage.
To prevent allergen- or exercise-induced bronchospasm:
- Adults: 2 puffs taken 15 minutes before exposure to allergens or exercise. Maximum 2 puffs, up to 4 times daily.
- Children: Half the adult dosage.
For chronic therapy:
- Adults: Up to 200 mcg (2 puffs) four times daily.
- Children: Up to 200 mcg (2 puffs) four times daily.
Note: Always use medications as directed by a licensed healthcare professional.
Proper Inhaler Use
Correct inhaler technique is essential for effective medication delivery. Improper use reduces drug efficacy. Follow these steps to ensure optimal inhaler usage:
- Remove the cap.
- Shake the inhaler vigorously at least six times before each use.
- If unused for a week or more, release one test puff into the air.
- Exhale fully and comfortably, then position the inhaler upright.
- Place the mouthpiece between your teeth, sealing your lips around it.
- Inhale deeply and slowly while pressing the canister firmly to release the medication.
- Hold your breath for at least 10 seconds (or as long as comfortable), then remove the inhaler.
- If prescribed multiple puffs, wait one minute between puffs and repeat steps 4–7.
- After use, replace the cap and rinse your mouth with water.
Pro Tip: Practice in front of a mirror occasionally. Visible mist indicates incorrect technique.
Inhaler Cleaning:
Clean weekly by removing the metal canister and rinsing the plastic actuator and cap with warm water. Let them air dry completely before reassembly.
Interactions
Avoid using Brodil with non-selective beta-blockers like propranolol, as they can reduce effectiveness. β2-agonists may cause significant hypokalemia, especially when combined with xanthine derivatives, steroids, or diuretics. Monitor potassium levels in acute asthma episodes.
Contraindications
Do not use Brodil Inhaler if allergic to salbutamol or any of its components.
Side Effects
Brodil is generally well tolerated, but mild side effects like tremors, anxiety, muscle cramps, headache, palpitations, and throat irritation may occur. Rare cases include tachycardia or cardiac arrhythmias.
Special Precautions
- Paradoxical Bronchospasm: Discontinue immediately if this rare, life-threatening condition occurs.
- Cardiovascular Effects: Monitor for pulse or blood pressure changes, particularly in patients with pre-existing heart conditions.
- Hypersensitivity Reactions: Discontinue if symptoms like rash, bronchospasm, or anaphylaxis develop.
- Coexisting Conditions: Use cautiously in patients with convulsions, hyperthyroidism, or diabetes.
Use in Pregnancy & Lactation
- Pregnancy: Category C. Use only if benefits outweigh potential risks.
- Lactation: Unclear if excreted in human milk. Administer with caution.
Storage Guidelines
- Store below 30°C. Protect from sunlight and heat.
- Do not puncture or burn the canister.
- Keep out of reach of children.
