Indications
Brodil serves as a bronchodilator and is recommended for:
- Asthma
- Chronic Bronchitis
- Emphysema
- Other airway obstruction-related conditions
Pharmacology
Salbutamol, a synthetic sympathomimetic agent, primarily targets beta-2 adrenergic receptors. By stimulating these receptors in bronchial smooth muscle, Salbutamol induces bronchodilation, leading to enhanced pulmonary function, as evidenced by spirometric assessments.
Dosage & Administration
Salbutamol Tablets or Syrup:
- Children:
- Ages 2-6: 2.5 ml syrup, 3-4 times daily
- Ages 6-12: 5 ml syrup, 3-4 times daily
- Over 12 years: 5-10 ml syrup, 3-4 times daily (2-4 mg tablet, 3-4 times daily)
- Adults: 2-4 mg tablet, 3-4 times daily (maximum single dose: 8 mg)
Salbutamol Respiratory Solution:
Use with a nebulizer under physician supervision. Do not inject or ingest.
Method 1: Intermittent Administration
- Adults (including elderly): 0.5-1.0 ml up to four times daily; may be increased to 40 mg/day under strict medical supervision.
- Children under 12 years: Start with 0.5 ml, diluted to 2.0-4.0 ml with normal saline. Some may require up to 1.0 ml.
Method 2: Continuous Administration
- Dilute 1-2 ml of solution to 100 ml with normal saline for continuous nebulization. Oxygen may be added to the inspired air if hypoventilation is a concern.
Salbutamol Injection:
- Adults: Dilute 5 mL of Salbutamol I.V. infusion solution (1000 mcg/mL) in 500 mL of a compatible IV solution to achieve a concentration of 10 mcg/mL. Infusion rates can start at 5 mcg/min and adjusted as necessary.
- Children and Adolescents: Currently, dosage for pediatric patients is not established.
Salbutamol Inhalation Capsules:
- Adults: For acute bronchospasm relief, one or two capsules as needed. Maintenance dosage: one 200 mcg capsule every 4-6 hours, with a maximum of two capsules if required.
- Children (4 years and older): One capsule for acute bronchospasm, taken 3-4 times daily. Can increase to two capsules if needed.
Exercise-Induced Asthma
- Adults: 400 micrograms, taken 15-30 minutes before exercise.
- Children: 200 micrograms under the same conditions.
Note: Always follow your healthcare provider’s advice regarding medication.
Side Effects
Common side effects of Brodil may include fine tremors (especially in the hands), palpitations, and muscle cramps. High doses may lead to tachycardia, headaches, and peripheral vasodilation.
Pregnancy & Lactation
Use during pregnancy only if the benefits outweigh potential risks. The excretion of Salbutamol in breast milk is unknown; consult with a healthcare provider to weigh the importance of the medication against the need for breastfeeding.
Precautions & Warnings
Brodil should be used cautiously in patients with hyperthyroidism, cardiovascular diseases, hypertension, and diabetes. Monitor potassium levels, as high doses can increase the risk of cardiac arrhythmias.
Overdose Effects
Symptoms may include angina, headache, nausea, tremors, and vomiting. Cardio-selective beta-blockers are the preferred antidote, but use with caution in patients with a history of bronchospasm.
Therapeutic Class
Short-acting selective β2-adrenoceptor stimulants.
Storage Conditions
Store below 30°C, away from light and moisture. Keep out of reach of children.
