Indications
Azithrocin is prescribed for treating infections caused by susceptible organisms. It is effective in:
- Lower respiratory tract infections such as bronchitis and pneumonia
- Upper respiratory tract infections including sinusitis, pharyngitis, and tonsillitis
- Otitis media (middle ear infections)
- Skin and soft tissue infections
- Sexually transmitted diseases like non-gonococcal urethritis and cervicitis caused by Chlamydia trachomatis.
Pharmacology
Azithromycin remains stable in acidic environments, making it suitable for oral administration without requiring protection from gastric acids. It is well-absorbed, with peak concentrations reached faster on an empty stomach. Azithromycin is actively transported to infection sites due to its high concentration in phagocytes. It exhibits a prolonged half-life, allowing for effective bacteriostatic levels in infected tissues for several days. The drug is primarily eliminated through bile, with a small percentage excreted unchanged in urine.
Microbiology
Azithromycin inhibits microbial protein synthesis by binding to the 50S ribosomal subunit of susceptible organisms. It has demonstrated activity against various bacteria and microorganisms, including gram-positive and gram-negative bacteria, Chlamydia spp., Mycoplasma spp., and others.
Dosage
For adults, oral dosages typically involve a 500 mg initial dose followed by 250 mg daily for varying durations depending on the condition. Children’s dosages are weight-based. Azithromycin can also be administered intravenously for certain infections.
Administration
To prepare the suspension, shake the bottle well and add boiled, cooled water to the marked level on the label. Shake until the powder completely dissolves.
Interactions
Azithromycin may interact with certain medications such as antacids, cyclosporine, digoxin, and others. Careful monitoring or dosage adjustments may be necessary when co-administered.
Contraindications
Azithromycin is contraindicated in patients with hypersensitivity to macrolide antibiotics or any of its components, as well as those with hepatic diseases.
Side Effects
Side effects of Azithrocin are generally mild and include gastrointestinal symptoms like nausea, abdominal pain, vomiting, and diarrhea. Rare allergic reactions and reversible elevations in liver enzymes have been reported.
Pregnancy and Lactation
Azithromycin is classified as Pregnancy Category B, indicating no evidence of fetal harm based on animal studies. Caution is advised when used during lactation.
Precautions & Warnings
Special precautions should be taken in patients with renal impairment. Azithromycin should not be used in patients with severe hepatic impairment. It is unlikely to affect the ability to drive or operate machinery.
Overdose Effects
Symptoms of overdose with macrolide antibiotics may include severe nausea, vomiting, diarrhea, and hearing loss. General supportive measures are recommended.
