Aldocort is a synthetic adrenocortical steroid with potent mineralocorticoid properties and strong glucocorticoid activity. It is primarily used for partial replacement therapy in conditions like Addison’s disease and salt-losing adrenogenital syndrome. The pharmacological action of Aldocort closely mirrors hydrocortisone, though it has enhanced and prolonged effects on electrolyte balance and carbohydrate metabolism. It works by acting on the kidneys to enhance sodium reabsorption, while promoting the excretion of potassium and hydrogen ions.
Pharmacology:
Fludrocortisone, the active ingredient in Aldocort, binds to the mineralocorticoid receptor (aldosterone receptor). This binding increases ion and water transport, which raises extracellular fluid volume and blood pressure, while simultaneously lowering potassium levels in the body.
Dosage & Administration:
- For Primary & Secondary Adrenocortical Insufficiency (Addison’s Disease): The typical dose ranges from 0.2 mg three times a week to 0.2 mg daily. If hypertension develops, reduce the dose to 0.05 mg daily. This is usually combined with cortisone or hydrocortisone therapy.
- For Salt-Losing Adrenogenital Syndrome: The usual dose is between 0.1 to 0.2 mg per day.
- For Postural Hypotension: The recommended dose is 0.1–0.4 mg daily for diabetic patients and 0.05–0.2 mg daily for those with postural hypotension secondary to Levodopa therapy.

Drug Interactions:
Aldocort may interact with the following medications:
- Amphotericin B, potassium-depleting diuretics, anticholinesterases, anticoagulants, antidiabetic drugs, and others.

Contraindications:
Aldocort should not be used in patients with:
- Known hypersensitivity to Fludrocortisone
- Systemic fungal infections
- Diabetic mellitus, osteoporosis, or acute infections
Additionally, caution is required when used in non-endocrine diseases requiring pharmacological doses.
Side Effects:
Common side effects due to the mineralocorticoid action of Aldocort (sodium and water retention) include:
- Erythema, purpura, vertigo, pancreatitis, and increased intraocular pressure
- Muscular weakness, hypertension, edema, and cardiac enlargement
- Osteoporosis, peptic ulcers, convulsions, and menstrual irregularities
When used at recommended dosages, side effects are typically rare. However, prolonged use or concurrent glucocorticoid therapy may increase the risk of adverse reactions.
Pregnancy & Lactation:
- Pregnancy: Category C. No adequate studies in pregnant women; use only if the benefits outweigh potential risks.
- Lactation: Fludrocortisone may pass into breast milk and could affect nursing infants. Caution is advised when using Aldocort during breastfeeding.
Precautions & Warnings:
Aldocort should be used cautiously in patients with:
- Active infections (e.g., tuberculosis, herpes, measles)
- Congestive heart failure, hypertension, renal insufficiency, osteoporosis, or peptic ulcer disease
Due to its effect on sodium retention, Aldocort is not recommended for conditions other than those indicated.
