Indications
Ace XR serves as a remedy for a variety of conditions including fever, common cold, influenza, headache, toothache, earache, body ache, myalgia, neuralgia, dysmenorrhoea, sprains, colic pain, back pain, post-operative pain, postpartum pain, inflammatory pain, and post-vaccination pain in children. It is also effective against rheumatic and osteoarthritic pain and stiffness of joints.
Pharmacology
Paracetamol, the active ingredient in Ace XR, possesses analgesic and antipyretic properties, along with mild anti-inflammatory activity. By primarily targeting the central nervous system (CNS), Paracetamol raises the pain threshold by inhibiting various isoforms of cyclooxygenase (COX) enzymes, including COX-1, COX-2, and COX-3, involved in prostaglandin synthesis. Paracetamol, a derivative of para-aminophenol, stands out as one of the most widely used, safe, and fast-acting analgesics, well-tolerated and devoid of the typical side effects associated with aspirin.
Dosage & Administration
- Tablet:
- Adults: Take 1-2 tablets every 4 to 6 hours, with a maximum of 4 grams (8 tablets) daily.
- Children (6-12 years): Administer ½ to 1 tablet 3 to 4 times daily. For prolonged treatment, it is advisable not to exceed 2.6 grams/day.
- Extended Release Tablet:
- Adults & Children over 12 years: Consume two tablets every 6 to 8 hours, not exceeding 6 tablets within a 24-hour period. Tablets should not be crushed.
- Syrup/Suspension: (Dosage varies based on age)
- Children under 3 months: 10 mg/kg body weight, administered 3 to 4 times daily.
- 3 months to below 1 year: ½ to 1 teaspoonful 3 to 4 times daily.
- 1-5 years: 1-2 teaspoonful 3 to 4 times daily.
- 6-12 years: 2-A teaspoonful 3 to 4 times daily.
- Adults: 4-8 teaspoonful 3 to 4 times daily.
- Suppository: (Dosage varies based on age)
- Children 3-12 months: 60-120 mg, 4 times daily.
- Children 1-5 years: 125-250 mg, 4 times daily.
- Children 6-12 years: 250-500 mg, 4 times daily.
- Adults & children over 12 years: 0.5-1 gm, 4 times daily.
- Pediatric Drop: (Dosage varies based on age)
- Children up to 3 months: 0.5 ml (40 mg).
- 4 to 11 months: 1.0 ml (80 mg).
- 7 to 2 years: 1.5 ml (120 mg). Do not exceed more than 5 doses daily for a maximum of 5 days.

Interaction
Certain medications and substances, such as barbiturates, tricyclic antidepressants, and alcohol, may affect the metabolism of Ace XR. Alcohol consumption can exacerbate the hepatotoxic effects of Ace XR overdose. Chronic use of anticonvulsants or oral steroid contraceptives may impact Ace XR’s therapeutic levels by inducing liver enzymes.
Contraindications
Ace XR should not be used by individuals with a known hypersensitivity to Paracetamol.
Side Effects
While side effects of Ace XR are typically mild, some individuals may experience hematological reactions such as thrombocytopenia, leucopenia, pancytopenia, neutropenia, and agranulocytosis. Occasional occurrences of pancreatitis, skin rashes, and allergic reactions have also been reported.
Pregnancy & Lactation
Studies suggest no adverse effects from Paracetamol when used within recommended doses during pregnancy. However, patients are advised to consult their healthcare provider before use. Paracetamol is excreted in breast milk but not in clinically significant amounts, thus considered safe for breastfeeding mothers.
Precautions & Warnings
Special care should be taken when administering Ace XR to patients with severe renal or hepatic impairment. Overdose risk is higher in individuals with non-cirrhotic alcoholic liver disease. Patients should avoid concurrent use of other Ace XR-containing products unless directed otherwise.
Overdose Effects
Liver damage is a concern with Ace XR overdose, especially with ingestion of 10 grams or more. Symptoms may include pallor, nausea, vomiting, anorexia, abdominal pain, and in severe cases, hepatic failure leading to various complications. Immediate treatment is crucial, including measures such as activated charcoal administration and N-acetylcysteine therapy within the appropriate timeframe.
