Arbitel Plus 80

Indications

Arbitel Plus is prescribed for the management of hypertension, either as a monotherapy or in combination with other antihypertensive medications. Effectively lowering blood pressure can significantly reduce the risk of both fatal and non-fatal cardiovascular events, such as strokes and heart attacks.

Pharmacology

Telmisartan: This medication is an angiotensin II receptor blocker (ARB) that prevents the vasoconstriction and aldosterone release effects of angiotensin II by selectively blocking its AT1 receptor. This results in blood vessel dilation and reduced blood pressure without affecting heart rate. Unlike other ARBs, Telmisartan has a much stronger affinity for the AT1 receptor, promoting greater blood pressure control.

Hydrochlorothiazide: A thiazide diuretic, it reduces sodium and chloride reabsorption in the kidneys, thus increasing their excretion. This leads to reduced plasma volume, lower blood pressure, and increased potassium loss, which can be counterbalanced by the use of ARBs like Telmisartan.

Dosage & Administration

• For patients inadequately controlled with Telmisartan alone (80 mg), initiate Arbitel Plus 80 mg/12.5 mg daily. The dose can be increased to 160 mg/25 mg after 2-4 weeks if needed.
• For those with hypokalemia on Hydrochlorothiazide: Switch to Arbitel Plus 80 mg/12.5 mg daily, with potential dose adjustments after 2-4 weeks.
• Arbitel Plus can also be used in combination with other antihypertensive drugs.

Interactions

Arbitel Plus may interact with drugs such as aliskiren, digoxin, lithium, NSAIDs, corticosteroids, ACE inhibitors, and other ARBs. Always consult your doctor for potential drug interactions before starting treatment.

Contraindications

Arbitel Plus is contraindicated in patients with hypersensitivity to its ingredients, pregnancy, lactation, or severe kidney or liver issues. It should not be combined with Aliskiren in diabetic patients.

Side Effects

Common side effects may include dizziness, headache, fatigue, nausea, and diarrhea. Other possible effects include muscle pain, insomnia, chest pain, and more. Contact your doctor if any unusual symptoms occur.

Pregnancy & Lactation

• Pregnancy Category D: Not recommended during pregnancy, especially in the second and third trimesters due to risks of fetal harm.
• Lactation: It’s unknown whether Arbitel Plus is excreted in breast milk, so weigh the risks versus benefits before breastfeeding.

Precautions & Warnings

Consult your healthcare provider if you have a history of kidney disease, liver disease, electrolyte imbalances, or gout. This medication may affect potassium levels and cause dizziness, so avoid activities requiring full alertness until you understand how it affects you.

Use in Special Populations

• Pediatric Use: Safety and effectiveness have not been established.
• Geriatric Use: Caution should be taken with elderly patients, especially those with hepatic or renal issues.
• Renal Impairment: Not recommended for severe renal impairment, though lower doses may be considered for mild to moderate conditions.

Overdose Effects

Overdosing on Arbitel Plus may result in low blood pressure, dizziness, electrolyte imbalances, and dehydration. Seek medical attention immediately.
