Description

Indications

Vave is indicated for:

  • Dyspeptic symptoms: Often linked to delayed gastric emptying, gastroesophageal reflux, and esophagitis, including epigastric fullness, abdominal distension, upper abdominal pain, nausea, vomiting, eructation, flatulence, and early satiety.
  • Heartburn and regurgitation: Associated with gastric reflux.
  • Acute nausea and vomiting: Due to functional, organic, infectious causes, dietetic origin, or induced by radiotherapy, drug therapy, or migraine.
  • Parkinson’s disease: Helps with nausea and vomiting caused by dopamine agonists.
  • Radiological studies: Speeds up barium transit in follow-through radiological imaging.

Pharmacology

Domperidone, a dopamine antagonist, works by blocking dopamine receptors in the stomach and Chemoreceptor Trigger Zone (CTZ). Its gastroprokinetic action helps restore gastrointestinal motility, enhance gastric emptying, and regulate pyloric contraction. Due to minimal penetration of the blood-brain barrier, Domperidone avoids psychotropic side effects. It also prevents regurgitation by increasing esophageal peristalsis and lower esophageal sphincter pressure.


Dosage & Administration

Domperidone should be taken 15-30 minutes before meals and before bedtime if needed.

Adults: 10-20 mg (1-2 tablets or 10-20 ml suspension) every 6-8 hours. Maximum dose: 80 mg daily.
Children: 2-4 ml suspension or 0.4-0.8 ml paediatric drops per 10 kg body weight, every 6-8 hours.

In dyspeptic symptoms:

  • Adults: 10-20 mg every 6-8 hours.
  • Children: 0.2-0.4 mg/kg body weight every 6-8 hours.

In acute conditions (e.g., nausea, vomiting):

  • Adults: 20 mg every 6-8 hours.
  • Children: 0.2-0.4 mg/kg every 6-8 hours. Maximum treatment duration: 12 weeks.

Rectal suppositories:

  • Adults: 30-60 mg every 4-8 hours.
  • Children: Maximum daily dose of 30 mg (for 10-25 kg weight).

Interactions

  • Anticholinergic drugs may reduce Domperidone’s effectiveness.
  • Antacids and antisecretory drugs lower Domperidone’s bioavailability.
  • CYP3A4 inhibitors (e.g., azole antifungals, macrolide antibiotics) may increase plasma levels of Domperidone.

Domperidone may alter the absorption of sustained-release or enteric-coated drugs. Always consult a healthcare provider for drug interactions.


Contraindications

  • Hypersensitivity to Domperidone.
  • Gastrointestinal obstruction, perforation, or hemorrhage.
  • Prolactin-releasing pituitary tumor (prolactinoma).

Side Effects

  • Rare side effects include transient intestinal cramps.
  • Possible increase in prolactin levels, leading to galactorrhea and gynaecomastia.
  • Rare neurological effects (e.g., extrapyramidal symptoms) in children.

Pregnancy & Lactation

  • Domperidone should only be used during the first trimester if the benefits outweigh potential risks.
  • Excreted in breast milk; not recommended for breastfeeding unless deemed necessary by a doctor.

Precautions & Warnings

  • Use with caution in children due to the risk of extrapyramidal reactions.
  • Caution in patients with liver impairment.
  • For prolonged use, regular reviews are recommended.

Use in Special Populations

  • Infants: Exercise caution due to an underdeveloped blood-brain barrier and potential neurological side effects.
  • Liver disorders: Caution in patients with hepatic impairment.
  • Kidney disorders: Adjust dosing frequency for severe renal insufficiency.

Overdose Effects

Overdose may cause drowsiness, disorientation, and extrapyramidal reactions, especially in children. Activated charcoal and close observation are recommended for treatment.
