Indications
Acilog Mix (rDNA) injection is prescribed to help control blood sugar levels in adults and children with type 1 diabetes mellitus, and in adults with type 2 diabetes mellitus.

Composition
Each 1 ml of the suspension contains Insulin Aspart (rDNA) 100 units (equivalent to 3.5 mg), consisting of 30% soluble Insulin Aspart and 70% Insulin Aspart Protamine suspension.
Pharmacology
Acilog Mix (rDNA) combines 30% Insulin Aspart and 70% Insulin Aspart Protamine (rDNA), an insulin analog that lowers blood glucose. It works faster than regular insulin and has an intermediate duration of action. Produced through recombinant DNA technology using Saccharomyces cerevisiae (baker’s yeast), this formulation is a modification of regular human insulin. The main function of Insulin Aspart is to regulate glucose metabolism by promoting glucose uptake in muscles and fat tissue and inhibiting the liver’s glucose production. It also prevents fat and protein breakdown and supports protein synthesis.
Dosage
The insulin aspart dosage must be personalized based on the individual needs of the patient.
- Subcutaneous Injection: Typically, insulin aspart should be injected 5-10 minutes before eating.
- Use in Pumps: Change the insulin in the pump reservoir every 6 days. Replace the infusion set and its insertion site at least every 3 days. Do not mix insulin aspart with other insulins or diluents when used in a pump.
- Intravenous Use: Insulin aspart can be diluted to concentrations between 0.05 U/mL and 1.0 U/mL in polypropylene infusion bags. It is stable in 0.9% sodium chloride infusion solutions.

Administration
Before administering Biphasic Insulin Aspart (rDNA), please ensure the following:
- Use the correct insulin syringe (100 units/ml).
- Confirm the insulin type as prescribed.
- Check the expiration date on the vial.
- Ensure the flip-off cap on the vial is intact.
Instructions for Use:
- Wash your hands thoroughly.
- Shake or roll the vial 10 times to mix the insulin thoroughly.
- Inspect the vial, which should appear cloudy and white after mixing.
- Remove the plastic cap and clean the vial’s rubber stopper with an alcohol swab.
- Draw air into the syringe matching your insulin dose.
- Insert the needle into the vial and push the plunger to inject air into the vial.
- Turn the vial and syringe upside down, and gently shake it. Ensure the needle tip is submerged in the liquid, then withdraw the correct insulin dose.
- Check for air bubbles in the syringe, as they reduce insulin dosage. If present, tap the syringe to float them to the top and expel them.
- Pinch the skin lightly and inject the insulin by pressing the plunger slowly. Ensure the needle is fully inserted.
- Wait 5 seconds before removing the needle, and avoid rubbing the injection site.

Interactions
Certain substances can influence blood glucose levels and may require dose adjustments and careful monitoring. Examples include:
- Increase the blood glucose-lowering effect: Oral anti-diabetic drugs, pramlintide, ACE inhibitors, diuretics, fluoxetine, and others.
- Decrease the blood glucose-lowering effect: Corticosteroids, diuretics, sympathomimetics, and certain other medications.
- Variable effects: Beta-blockers, clonidine, lithium, and alcohol may either increase or decrease insulin’s effect.
Contraindications
Acilog Mix (rDNA) should not be used in the following situations:
- During hypoglycemic episodes.
- In patients who have a hypersensitivity to Insulin Aspart or any of its ingredients.
Side Effects
Common side effects of Insulin Aspart include hypoglycemia, lipodystrophy, weight gain, and peripheral edema.
Pregnancy & Lactation
- Pregnancy: Category B. Insulin needs may fluctuate during pregnancy, so blood glucose should be closely monitored. Pregnant women should inform their healthcare provider if they become pregnant while using insulin aspart.
- Lactation: It is unknown if Insulin Aspart passes into breast milk. Breastfeeding women may need to adjust their insulin dosages.
Precautions & Warnings
- Blood Glucose Monitoring: Regular monitoring is essential for all patients using insulin. Adjustments to insulin regimens should only be made under medical supervision.
- Renal or Hepatic Impairment: Dose adjustments may be required for patients with kidney or liver issues.
Use in Special Populations
- Pediatrics: Insulin Aspart has not been studied in children under 2 years of age or those with type 2 diabetes.
Overdose Effects
Excessive insulin aspart may lead to hypoglycemia. Mild cases can be treated with oral carbohydrates, while severe cases may require intravenous glucose or glucagon injections.
