Indications
Ocular Use: This medication is recommended for managing inflammatory eye conditions responsive to steroids, particularly when there is a risk of bacterial infection or an existing infection. It is also effective for treating post-operative inflammation and other ocular inflammations linked to infections.
Aural Use: This combination is suitable for treating ear infections accompanied by inflammation, including conditions such as otitis externa and both acute and chronic otitis media. Additionally, it can be utilized post-operatively in ear procedures.

Pharmacology
Dexamethasone, a glucocorticoid, exhibits potent anti-inflammatory and anti-allergic properties. When applied topically, it effectively alleviates inflammatory conditions affecting the anterior segment of the eye by reducing prostaglandin synthesis through the inhibition of phospholipase A2. Furthermore, Dexamethasone minimizes the chemotactic migration of neutrophils to the site of inflammation.
Ciprofloxacin demonstrates in vitro effectiveness against a broad spectrum of gram-negative and gram-positive bacteria, showcasing the highest antibacterial potency among quinolones. Its bactericidal effect arises from the disruption of the DNA gyrase enzyme, crucial for bacterial DNA synthesis.
Dosage & Administration
For Ocular Use: Instill 1 drop into the conjunctival sac(s) every four to six hours. During the first 24 to 48 hours, the dosage may be increased to 1 drop every two hours.
For Aural Use:
- Acute Otitis Media in Pediatric Patients with Tympanostomy Tube: Administer 4 drops into the affected ear twice daily for 7 days.
- Acute Otitis Externa: Instill 4 drops into the affected ear twice daily for 7 days.
Pediatric Use: The safety and efficacy of this medication in children under 1 year have not been established.

Interactions
While specific studies on interactions between ophthalmic Ciprofloxacin and Dexamethasone are lacking, systemic administration of certain quinolones has been known to elevate plasma theophylline levels, alter caffeine metabolism, enhance the effects of oral anticoagulants like warfarin, and may lead to temporary increases in serum creatinine in patients taking cyclosporin concurrently.
Contraindications
This medication should not be used in individuals with hypersensitivity to any of its components. It is contraindicated in cases of herpes simplex and other viral infections, mycosis, glaucoma, in newborns, and fungal infections affecting the ocular or auricular regions.
Side Effects
Ocular: Commonly reported adverse effects associated with Ciprofloxacin include temporary ocular burning or discomfort. Other possible reactions are stinging, redness, itching, conjunctivitis/keratitis, swelling around the eyes, foreign body sensation, photophobia, blurred vision, tearing, dryness, and eye pain. Rare instances of dizziness have also been noted. Steroid-related effects may include increased intraocular pressure (IOP), which could lead to glaucoma, optic nerve damage, posterior subcapsular cataract formation, and delayed wound healing.
Aural: The most frequent side effects are discomfort and ear pain, along with irritability, dizziness, and erythema.
Pregnancy & Lactation
Pregnancy: Currently, there are no well-controlled studies regarding the use of this medication in pregnant women. It should only be administered during pregnancy if the potential benefits outweigh any risks to the fetus.
Nursing Mothers: It remains unclear whether this drug passes into human milk; therefore, caution is advised when administering this medication to nursing mothers.
Precautions & Warnings
Consider the risk of fungal infections in the cornea following prolonged steroid use, especially in patients with persistent corneal ulcers. Long-term antibiotic use may lead to the overgrowth of non-susceptible organisms. Prolonged use of topical ophthalmic steroids can elevate intraocular pressure, potentially resulting in optic nerve damage, visual acuity defects, field vision loss, and posterior subcapsular cataract formation. Regular monitoring of intraocular pressure is recommended.
