Indications
Trupan is prescribed for conditions where reducing stomach acid is beneficial, including:
- Peptic ulcer diseases
- Gastroesophageal reflux disease (GERD)
- Ulcers caused by non-steroidal anti-inflammatory drugs (NSAIDs)
- Eradication of Helicobacter pylori (in combination with antibiotics)

Pharmacology
Pantoprazole, the active ingredient in Trupan, is a proton pump inhibitor that reduces gastric acid production. It binds covalently to the H+/K+ ATPase enzyme system on the surface of gastric parietal cells, effectively inhibiting both basal and stimulated acid secretion for over 24 hours.
Dosage & Administration
Oral Administration:
- Benign Gastric Ulcer: 40 mg once daily in the morning for 4 weeks; extend another 4 weeks if necessary.
- Gastroesophageal Reflux Disease (GERD): 20-40 mg once daily in the morning for 4 weeks; continue for another 4 weeks if required. Maintenance dose: 20 mg daily, can be increased to 40 mg daily.
- Duodenal Ulcer: 40 mg once daily in the morning for 2 weeks; extend another 2 weeks if needed.
- Helicobacter pylori-associated Duodenal Ulcer: 40 mg twice daily along with Amoxicillin 1 g and Clarithromycin 500 mg, both twice daily for one week.
- NSAID-associated Gastric or Duodenal Ulcer Prophylaxis: 20 mg daily for long-term NSAID users.
- Zollinger-Ellison Syndrome: Start with 80 mg once daily; adjust based on response (maximum 40 mg daily in elderly); doses above 80 mg should be divided.
IV Injection:
- Duodenal and Gastric Ulcer: 40 mg once daily for 7-10 days.
- GERD with Erosive Esophagitis History: 40 mg once daily for 7-10 days.
- Peptic Ulcer Bleeding Prevention: IV 80 mg, then 8 mg/hour infusion for 72 hours.
- Acid Aspiration Prophylaxis: 80 mg IV every 12 hours for 24 hours, followed by 40 mg every 12 hours.
- Long-term Zollinger-Ellison Syndrome Management: 80 mg IV every 12 hours; may increase to 80 mg every 8 hours if needed.

Contraindications
Trupan should not be used by individuals with a known hypersensitivity to its components.
Side Effects
Trupan is generally well-tolerated. Common side effects include headache and diarrhea. Rare side effects include abdominal pain, flatulence, rash, insomnia, and hyperglycemia.
Pregnancy & Lactation
Pantoprazole is classified as a Category B drug by the US FDA. While no well-controlled studies exist for pregnant women, it should be used during pregnancy only if necessary. Pantoprazole is excreted in breast milk, so a decision should be made to either discontinue nursing or the drug.
Precautions & Warnings
Patients should avoid splitting, chewing, or crushing Trupan tablets. Long-term use may lead to Vitamin B12 deficiency or increase the risk of osteoporosis-related disorders.
Overdose Effects
No specific overdose symptoms are known. Since Trupan is highly protein-bound, it is not easily dialyzable. Symptomatic and supportive treatment is recommended.

IV Injection:
- Use 10 ml of 0.9% Sodium Chloride Injection to reconstitute the lyophilized powder. Administer as a slow intravenous injection over at least 2 to 5 minutes. The reconstituted solution can be stored at room temperature for up to 4 hours.
IV Infusion:
- Reconstitute with 10 ml of 0.9% Sodium Chloride Injection and further dilute to 100 ml with 0.9% Sodium Chloride Injection, 5% Dextrose, or Lactated Ringer’s Injection. The solution should be used within 24 hours after initial reconstitution.
