Indications
Xten is prescribed for treating painful inflammatory and degenerative musculoskeletal conditions including:
- Rheumatoid arthritis
- Osteoarthritis
- Arthrosis
- Ankylosing spondylitis
- Extra-articular disorders such as tendinitis, bursitis, periarthritis (shoulder-hand syndrome), hip periarthritis, strains, and sprains
- Acute gout
Pharmacology
Tenoxicam, a non-steroidal anti-inflammatory drug (NSAID), possesses anti-inflammatory, analgesic, antipyretic properties, and inhibits platelet aggregation. It reduces prostaglandin synthesis and acts as a scavenger for active oxygen during inflammation. Tenoxicam also inhibits human metalloproteinases, which are involved in cartilage breakdown, contributing to its therapeutic effects in musculoskeletal disorders. Animal studies showed no mutagenic, carcinogenic, or teratogenic effects.
Dosage
- Standard dosage: 20 mg once daily for all indications except acute gout.
- Acute gout: 40 mg once daily for 2 days, followed by 20 mg once daily for 5 days.
- Chronic disorders: Early therapeutic effects are evident with a progressive increase over time. Daily doses exceeding 20 mg should be avoided unless necessary for long-term treatment, where a reduction to 10 mg daily may be considered.
Administration
Take tablets with a glass of water, preferably during or immediately after a meal.
Interactions
Xten may interact with salicylates, NSAIDs, methotrexate, lithium, and certain diuretics. Careful monitoring is advised when co-administered with these medications.
Contraindications
Do not administer Xten to patients with hypersensitivity to tenoxicam, a history of asthma, rhinitis, or urticaria induced by salicylates or other NSAIDs, or upper gastrointestinal tract diseases like gastritis or ulcers.
Side Effects
Most reported side effects are mild and transient, including gastrointestinal discomfort, dizziness, and headache. Serious reactions, though rare, may include gastrointestinal bleeding, skin reactions, and liver enzyme abnormalities.
Pregnancy & Lactation
Avoid use during the third trimester of pregnancy due to potential fetal effects. Minimal amounts of tenoxicam pass into breast milk; caution is advised during lactation.
Precautions & Warnings
Monitor renal and cardiac function in patients at risk of renal failure or cardiovascular complications. Xten may inhibit platelet aggregation and should be used cautiously in patients with bleeding disorders.
Use in Special Populations
Not recommended for children under 16 years due to lack of established safety and efficacy data in this age group.
Overdose Effects
Conventional measures to reduce absorption and enhance elimination should be considered in case of overdose, although acute overdosage experience with Xten is limited.
Storage Conditions
Store below 30°C, protected from light and moisture, and keep out of reach of children.
