Indications
Xidolac Meltab is prescribed for short-term relief of moderate to severe post-operative pain. It is also effective for managing pain, inflammation, and light sensitivity following ocular surgery and can reduce eye inflammation or redness caused by seasonal allergies.
Description
Xidolac Meltab belongs to the pyrrolo-pyrrole class of nonsteroidal anti-inflammatory drugs (NSAIDs) and contains ketorolac. Known chemically as 5-benzoyle-2,3-dihydro-1H-pyrrolo[1,2-a]quinoline-1-carboxylic acid, it functions by inhibiting prostaglandin synthesis, which alleviates pain and inflammation. Xidolac Meltab is highly protein-bound, metabolized in the liver, and excreted through the urine.
Pharmacology
Ketorolac Tromethamine, the active component of Xidolac Meltab, is a powerful NSAID that mitigates pain by blocking the cyclooxygenase enzyme system, thus reducing prostaglandin production. While its anti-inflammatory effects are minimal at analgesic doses, it is highly effective for pain management.
Dosage & Administration
- Tablet: Take 10 mg every 4-6 hours, not exceeding 40 mg per day. For short-term use only (up to 7 days); not intended for long-term use.
- Injection: Administer IV or IM for moderate to severe pain, with initial doses as follows:
- IM Dosing:
- Adults <65 years: 60 mg
- Adults >65 years, renally impaired, or <50 kg: 30 mg
- IV Dosing:
- Adults <65 years: 30 mg
- Adults >65 years, renally impaired, or <50 kg: 15 mg
- Children (2-16 years):
- IM Dosing: 1 mg/kg, up to 30 mg
- IV Dosing: 0.5 mg/kg, up to 15 mg
- IM Dosing:
- Multiple-Dose Treatment:
- Adults <65 years: 30 mg every 6 hours, max 120 mg/day
- Adults >65 years, renally impaired, or <50 kg: 15 mg every 6 hours, max 60 mg/day
- Transition from parenteral to oral therapy should not exceed 5 days. The combined daily dose should not exceed 120 mg on the day of transition and 40 mg subsequently.
- Eye Drops: 1 drop in each eye, 4 times daily.
Drug Interactions
- Other NSAIDs/Aspirin: May increase side effects.
- Anti-coagulants: Can enhance anti-coagulant effects.
- Beta Blockers: May reduce anti-hypertensive efficacy.
- ACE Inhibitors: Can increase the risk of renal impairment.
- Methotrexate: May heighten toxicity.
Contraindications
Do not use Xidolac Meltab if allergic to ketorolac or other NSAIDs, in children under 16, or as a preventive measure before surgery.
Side Effects
Common side effects include nausea, vomiting, gastrointestinal bleeding, peptic ulcers, pancreatitis, drowsiness, headache, excessive thirst, fatigue, bradycardia, hypertension, palpitations, chest pain, infertility in women, and pulmonary edema.
Pregnancy & Lactation
Xidolac Meltab is classified as FDA pregnancy category C. It should be avoided during pregnancy and lactation unless the potential benefits justify the risks to the fetus.
Precautions & Warnings
Exercise caution in individuals over 65 years old, those with active or suspected peptic ulcers, gastrointestinal bleeding, asthma, or liver dysfunction. For eye drops, discontinue use if corneal epithelial damage is observed and avoid using while wearing contact lenses.
